Çocuklarda ve ergenlerde travma sonrası stres bozukluğu belirtilerinin tedavisinde fluoksetinin etkinliği

Özet
Amaç: Bu çalışmanın amacı, çocuk ve ergenlerde travma sonra-sı stres bozukluğu (TSSB) belirtilerinin tedavisinde fluoksetinin etkinliğini araştırmaktır. Yöntem: Çalışma, Türkiye'de yaşanan deprem felaketinden sonra, TSSB belirtileri nedeniyle başvuran, yaşları 7 ile 17 (ört: 13.0±2.8 /yıl) arasındaki, 26 çocuk ve ergen (10 erkek, 16 kız) ile yapıldı. TSSB tanısı DSM-IV ölçütlerine göre konuldu. Çalışma 30 ile 76 gün (ortalama 55*10.91 gün) sürdü. Günde 7-20mg (ört: 15.1±5.3 mg/gün) fluoksetin verildi. Fluoxetinin etkinliğinin araştırılmasında Klinik Genel Değerlen-dirme Ölçeği (KGDÖ) ile tarafımızdan geliştirilen Travma Sonra-sı Stres Bozukluğu Belirti Tarama Formu (TSSBBTF) kullanıldı. Bulgular: KGDÖ'ne göre olguların dokuzunda (%34.6) çok dü-zelme, on dördünde (%53.8) oldukça düzelme, birinde (%3.8) bi-raz düzelme, birinde (%3.8) hiç değişmeme, birinde (%3.8) ise kötüleşme gözlendi. TSSBBTF'na göre, fluoksetinin tüm TSSB belirtilerinde istatistiksel olarak anlamlı düzeyde iyileşmeye yol açtığı saptandı. Sonuç: Bu çalışmada fluoksetin tedavisinin ço-cuklarda ve ergenlerde deprem travması sonrasında meydana gelen; olayı yeniden yaşama, travmaya eşlik eden uyaranlardan kaçınma, tepki göstermede azalma ve uyarılmışlıkta artma belir-tilerinde etkili olduğu sonucuna varıldı.
Abstract
Objective: The aim of this study is to investigate the effectiveness of fluoxetine on the symptoms of posttraumatic stress disorder (PTSD) with children and adolescents. Method: This study was performed with 26 children and adolescents (10 males, 16 females), aged between 7 and 17 (mean: 13.0±2.8/year), beca-use of PTSD symptoms developed after earthquake disaster happened in Turkey. PTSD diagnosis was made according to DSM-IV. The Clinical Global Impression Scale, and the Posttraumatic Stress Disorder Symptoms Check List which was developed in our department were used to assess the effectiveness of 7-20mg (mean: 15.1 ±5.3 mg/day) fluoxetine treatment vvhich lasted betvveen 30 and 76 days (mean: 55±10.91days). Results: According to the Clinical Global Impression Scale, 9 subjects (34.6%) improved very much, 14 subjects (53.8%) improved much, 1 subjects (3.8%) improved slightly, 1 subjects (3.8%) did not change, and 1 subjects (3.8%) vvorsened. Fluoxetine caused statistically significant improvements on ali PTSD symptoms according to the Posttraumatic Stress Disorder Symptoms Check List. Conclusion: in this study, it was observed that fluoxetine was effective on symptoms of reexperiencing, of avoidance'of stimuli associated with trauma, of numbing of general responsiveness, and of increased arousal vvhich happened after earthquake trauma to the children and adolescents.
