Şizofrenili hastalarda haloperidol ve risperidon'un klinik etki ve yan etkileri

Özet
Amaç: Bu çalışmada, şizofrenili hastalarda tipik antipsikptik haloperidol ye atipik antipsikotik risperidon'un klinik etki ve yan etkilerinin karşılaştırılması amaçlanmıştır. Yöntem: DSM-IV tanı ölçütlerine göre şizofreni tanısı alarak servise yatırılan, gönüllü erkek hastalar çalışmaya alınmıştır. Hastalar geliş sırasına göre ardışık olarak risperidon ve haloperidol gruplarına ayrılmıştır. 10 günlük ilaçtan arındırma döneminden sonra deneklere ilaç başlanmıştır, ilaç dozu seçiminde esnek davranılmış fakat seçilen dozda devam edilmiştir. Sekiz hafta süreyle hastalara kullanılan antipsikotik ilaç günlük doz ortalamaları; haloperidol grubu için 22.75±5.15mg/gün, risperidon grubu için 6.40$pm$1.23mg/gün idi. ilaçtan arındırma döneminde lorazepam, tedavi periyodu boyunca biperiden; gereken olgularda kullanılmıştır. Her iki grupta yirmişer hasta tedavi süresini tamamladığında çalışma sonlandırılmıştır. Tüm deneklere başlangıçta sosyodemografik bilgiler ve hastalık öyküsüne yönelik özelliklerin soruşturulduğu yarıyapılandırılmış bir görüşme anketi uygulanmıştır. Klinik değerlendirme için tedaviden önce, tedavinin 1., 4. ve 8.nci haftaları sonunda Kısa Psikiyatrik Derecelendirme .Ölçeği (KPDÖ), Pozitif Belirtiler Derecelendirme Ölçeği (PBDÖ), Negatif Belirtiler Derecelendirme Alt Ölçeği (NBDÖ), Klinik Global İzlenim Ölçeği (CCI) hastalığın şiddeti ve Ekstrapiramidal Belirtileri Derecelendirme Ölçeği (EBDÖ); tedavinin 1,..4. ve 8.nci haftalarında ise UKU Yan Etkileri Derecelendirme Ölçeği uygulanmıştır. Bulgular: Her iki grupta da psikotik semptomlarda başlangıca göre anlamlı düzeyde bir iyileşme gözlendi. Abstract
Objective: In this study were aimed to compare the efficacy and safety of typical antipsychotic haloperidol and atypical antipsychotic risperidone in patients with schizophrenia. Method: Voluntary male inpatients who were diagnosed as schizophrenia according to DSM-IV criteria were recruit for the study. Patients were consecutively dichotomized into risperidone and haloperidol groups according to their order of hospital admittance. After 10 days of drug wash-out the patients were given the medications. Dosing has been flexible and a fix dose has been maintained. For 8 weeks the mean doses of drugs for the patients were 22.75$pm$5.15 mg/day for the haloperidol group, and 6.40$pm$1.23 mg/day for the risperidone group. Lorazepam has been used in the wash-out period, and biperidine per needed has been used during the treatment period. The study has been finished when 20 patients from each group have completed the treatment period. All subjects were given a sociodemographic questionnaire as well as a semi-structured interview concerning the features of the current and past psychiatric disorder. For clinical evaluation; before treatment and at the end of the 1 st, 4th and 8th weeks Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Positive Symptoms (SAPS), Scale for the Assesment of Negative Symtoms (SANS), Clinical Global Impression (CGI)-Severity of Illness Subscale, Extrapyramidal Symptom Rating Scale (ESRS); and also UKU Side-Effect Rating Scale at the 1st, 4th and 8th weeks at treatment were applied all of the subjects. Results: Both groups showed a significant improvement in psychotic symptoms compared to the beginning.
