The evaluation of renal toxicity induced cisplatin theraphy in oncologic patients

Özet
Özet; Sunulan çalışmada antineoplastik ajan olarak cysplatin (CISN) uygulanan 33 kanser hasta serumunda tedavi öncesinde ve tedavi sonrasında creatinin ve cystatin C düzeyleri ölçülerek cysplatine bağlı olası bir nefrotoxisite varlığı araştırıldı. Bu hastaların ortak özelliği, hepsinin tedavi protokolleri içerisinde 5 günlük 20 mg/gün dozunda bu ilacın bulunması ve her 3 haftada bir uygulanmasıydı. Hasta grubu yüksek grade ve stage sahip çeşitli türden tümü metastatik kanser hastalarından seçildi. Tüm vakalar üçüncü ve dördüncü kemoterapi kürü içinde olup uygulanan cysplatin dozu her hasta için aynı idi. Bulunan değerler gerek 25 tane tüm biyokimya değerlen normal olan kontrol grubu değerleriyle ve gerekse tedavi öncesi ve sonrasında elde edilen değerlerle kıyaslandı. Çalışma sonucunda elde edilen istatistiksel verilere göre, creatinin ve cystatin C düzeyleri kemoterapi öncesinde ve kemoterapinin altıncı günü sonunda kendi referans aralıklarında bulunmuştur. Buna göre bu dozdaki cysplatin ile herhangi bir böbrek toxisitesi gelişmemiş gibi görünmekte ancak, cystatin C düzeyinin CISN kemoterapisi sonunda öncesindeki değere göre anlamlı bir artış gösterdiği bulunmuştur. Dolayısıyla, renal toxisite yönünden izlem amacıyla, renal tubullerdeki minimal değişiklikleri göstermesi yönünden creatininden daha hassas olduğunu düşündüğümüz serum cystatin C tayinini önermekteyiz.
Abstract
Summary: In this the nephrotoxic effect of the antineoplastic agent Cisplatin administered 20 mg/dayfor 5 days every 3 weeks, was evaluated in sera of 33 oncologic patients. All of them had Cisplatin in the chemotheraphy protocol. Nephrotoxicity was assessed by the creatinin and cystatin-c assays and the values obtained before and after chemotheraphy were compared with those of the control group which composed of 25 sera with full normal biochemical values. The patients were selected from -those different malignant metastatic tumors with high grade and stage. All of the cases were in the course of 3th or 4lh chemotheraphy cycle and the Cisplatin doses administered were the same for each case. Though renal toxicity did not occur with this dose of Cisplatin in our patient group (creatinine and cystatin-c values were in the control ranges both before and after chemotheraphy), a significant increase in Cystatin C vaules was observed after chemotheraphy (p
