Alt gastointestinal cerrahide stoma tercihi ne olmalı?

Özet

Amaç: Bu çalışmada, İstanbul Bakırköy Dr. Sadi Konuk Eği- tim ve Araştırma Hastanesi Genel Cerrahi Kliniğinde son altı yılda alt gastrointestinal sistemde stoma uygulaması yapılan hastalar değerlendirildi. Gereç ve Yöntem: Mayıs 2005 - Mayıs 2011 tarihleri arasın- da alt gastrointestinal sistem hastalıkları nedeniyle acil ve elektif olarak stoma uygulanan 201 hasta retrospektif olarak değerlendirildi. Hastaların endikasyonu, tercih edilen stoma tipi, cerrahi komplikasyonlar, kapatılma süreleri ve kapatma ameliyatlarından sonra görülen komplikasyonlar geriye dö- nük olarak değerlendirildi. Bulgular: Stoma yapılan 201 hastanın %60ı erkek ve yaş or- talaması 70.5 (dağılım, 18-87 yaş) idi. Stoma uygulamasının %82.5i acil ameliyatlarda yapıldı. En sık endikasyon rekto- sigmoid malignite cerrahisi idi (%52). En sık tercih edilen sto- ma çeşidi uç kolostomi idi (%27). Tüm stomaların %10unda stenoz, iskemi, prolapsus, parastomal herni ve retraksiyon gibi komplikasyonlar görüldü. Stomaya ait komplikasyon- lar en sık uç kolostomide görüldü. Stomaların %43ü çeşit- li nedenlerle kapatılamadı. En sık görülen neden hastaların kapatılma öncesi hayatlarını kaybetmeleri idi (%53). Stoma- sı kapatılan hastaların %30unda komplikasyon gelişti. En sık görülen komplikasyon yara yeri enfeksiyonu idi ve en sık uç kolostomi kapatılması sonrasında görüldü. En az komplikas- yon saptırıcı tipte enterostomilerde izlendi. Sonuç: Stoma komplikasyon oranı yüksek bir işlemdir. Bu sebeple alt gastrointestinal sistem cerrahisinde primer anas- tamoz öncelikle düşünülmeli, stoma ihtiyacı olan hastalarda saptırıcı enterostomi tercih edilmelidir.

Abstract

Background: In this study, we aimed to make analysis of pa- tients in whom lower gastrointestinal system stomas were per- formed at General Surgery Clinic of Istanbul Bakirkoy Dr. Sadi Konuk Training and Research Hospital during last six years. Methods: Totally 201 patients in whom stoma was performed following emergency or elective lower gastrointestinal system surgery during May 2005 and May 2011 were retrospectively analyzed. Indication, stoma type, surgical complications of sto- ma, time to stoma closure and complications related to closure were evaluated. Results: Of the 201 patients, 60% were male. Mean age of the patients were 70.5 (range 18 to 87 years). Stomas were per- formed via emergency surgery in 82,5% of cases. Surgery for rec- to-sigmoid cancer was the leading indication (52%). End colos- tomy was preferred in 27% of the patients. Overall complication rate of 10% was observed related to stoma (stenosis, ischemia, prolapsus, parastromal hernia and retraction). The highest com- plication rate was in patients with end colostomy. Totally 43% of the stomas could not be closed due to several reasons where death before closure was the major cause (53%). A complication rate of 30% was observed in cases when stoma closure was per- formed. The most common complication which is wound infec- tion was observed in end colostomy closure. The lowest overall complication rate was in group of loop enterostomy patients. Conclusion: Stoma is a surgical procedure which has a rela- tively high complication rate. For this reason in lower gastroin- testinal tract surgery, primary anastomosis should be the first choice in general. In cases where stoma is needed, loop enter- ostomy should be preferred, when possible.
