Özet
Amaç: Kliniğimizde takip edilen pnömotoraks olgularının tedavi sonuçlarını araştırdık.Hastalar ve Yöntem: 1998 ile 2009 yılları arasında hastanemizde tedavi edilen 426 pnömotoraks olgusu geriye dönük olarak incelendi.Bulgular: Olguların 184 tanesi primer spontan, 156sı sekonder spontan, 84ü travmatik, 2si iyatrojenik pnömotorakstı. Otuz bir olguya hemotoraks da eşlik etmekteydi. Yaş aralığı 5-93 (ortalama 43.7) şeklinde idi. Hastaların % 87si erkek ve pnömotoraksın görüldüğü taraf %55.8 sağ, %43.2 soldu. Hastanede yatış süresi ortalama 8.3 (5-93) gündü. Olgularımızın çoğunluğunda tüp torakostomi/plevral kateter uygulaması yeterli iken, minimal pnömotoraksı (%10 ve altı) olan hastalarda konservatif davranıldı. Yirmi bir hastaya uzamış/masif hava kaçağı nedeniyle torakotomi yapıldı. Yirmi iki hastada nüks (%5) gelişti.Sonuç: Çalışmamızda periferde bulunan bir devlet hastanesinin göğüs cerrahi kliniğinin pnömotoraks tedavisindeki sonuçlarını sunduk. Nüks oranının ve torakotomi gereksiniminin az olması, pnömotoraks tedavisinde başarılı olduğumuzu göstermektedir.
Abstract
Background: We reviewed the results of pneumothorax cases treated in our clinic.Methods: Between 1998 and 2009, 426 pneumothorax cases treated in our hospital were analyzed retrospectively.Results: One-hundred-eighty-four cases were primary spontaneous, 156 secondary spontaneous, 84 traumatic, and 2 iatrogenic pneumothorax. Thirty-one cases had hemothorax at the same time. Age range was 5 to 93 (mean 43.7) years. 87% of patients were male and pneumothorax was on the right side in 55.8% and on the left side in 43.2%. The mean duration of stay in hospital was 8.3 (5-9.3) days. Tube thoracostomy/pleural catheter application were usually sufficient. Patients with minimal pneumothorax (10% or less) were treated conservatively. Thoracotomy was performed in 21 patients who had prolonged or massive air leaks. Recurrent diseases occured in 22 patients (5%).Conclusion: In our study, the results of the treatment of pneumothorax at a thoracic surgery clinic at a state hospital are presented. Low recurrence rate and reduced need for thoracotomy indicates successful treatment of pneumothorax.
