Özet

Amaç: İntertrokanterik femur kırığı (İTFK) nedeniyle 135°’lik di-namik kalça çivisi ile tedavi ettiğimiz hastalarda tip-apeks me-safesinin (TAD) ölçülerek sonuçlara etkisinin ve TAD değerininsıyrılma oranlarını saptamada anlamlı bir tahmini değer olupolmadığının araştırılması amaçlandı. Gereç ve Yöntem: 2000-2005 yılları arasında tedavi edilen 110 hasta (55 kadın, 55 erkek) araştırmaya alındı. Hastaların yaş or- talaması 69 idi. Kırıkların 20’si (%18) stabil, 90’ı (%82) instabil olarak değerlendirildi. ulgular: Kyle sınıflamasına göre 1 tip 1, 19 tip2, 85 tip3, 5 tip4kırık saptandı. Sıyrılma görülen hastaların yaş ortalaması 79 idi.Hepsinin kırıkları instabil kırıktı (Kyle tip 3). Tüm hastaların orta-lama TAD değeri 21.9 mm bulundu. Hastalardan 3’ünde (%2.7)sıyrılma saptandı. Bu hastaların ortalama TAD değeri 36.08 idi.Bu değerin altındaki hiçbir hastada sıyrılma görülmedi.Sonuç: TAD değerinin hem çivinin konumu hem de derinliğinihesaba kattığı, böylece tek bir değer elde edildiği ve bu değerindaha önemli bir tahmini değer olduğu saptandı. Bununla be-raber yine de tek belirteç olmadığı, bu değerle beraber kırığınstabil olması ve çivinin baş içindeki yerleşimi önemli bulundu.

Abstract

Background: We measured tip-apex distance (TAD) in patients with intertrochanteric femur fracture who were treated surgically with 135° dynamic hip screw. We evaluated whether TAD is an ap- propriate measurement to determine the risk of cut-out. Methods: 110 patients (55 females, 55 males) who were treated between 2000-2005 were included in this study. The mean age of the patients during operation was 69. Twenty of these fractures (18%) were stable, and 90 (82%) were unstable. Results: According to Kyle classification; there were 1 type 1, 19 type 2, 85 type 3, 5 type 4 fractures. The mean age of the patients in whom cut out of the screw was observed was 79. All of these pa- tients had unstable intertrochanteric fracture (Kyle type 3). The TAD averaged 21.9 mm. In the 3 femoral heads from which the screw had cut-out, TAD averaged 36.08 mm. None of the screws with an averaged TAD of 21.9 or less were cut-out. Conclusion: With use of TAD, both the location and depth of pen- etration of the screw are taken into account. However, TAD is not a unique predicting measurement. The stability of the fracture and placement of the screw in the femoral head are important factors in the results.
