Özet

Amaç: Bu çalışmada karsinoid tümör saptanan hastalar de- ğerlendirildi ve literatür eşliğinde apendiks karsinoid tümör tedavi yaklaşımı incelendi. Gereç ve Yöntem: Ocak 2009-Aralık 2011 yılları arasında 1071 apendektomi olgusunun histopatolojik incelemesin- de karsinoid tümör olarak rapor edilen olgular, klinik bulgu, ameliyat bulgusu, histopatoloji ve takipleri yönünden geriye dönük olarak incelendi. Bulgular: Çalışmaya, apendiks histopatolojik incelemesinde karsinoid tümör saptanan altı (%0.6) olgu alındı. Yaş ortala- ması 28 (dağılım 23-31 yıl) idi. Hastaların beşi erkek ve biri kadındı. Ameliyat sırasında hiçbir olguda tümöral bir olu- şum düşünülmedi, tüm olgular insidental karsinoid tümör olarak değerlendirildi. Ortalama tümör boyutu 0.5 cm (dağı- lım 0.1-1.2 cm) idi. Ki-67, dört hastada incelendi ve tüm has- talarda %2nin altında idi. Yatış süreleri ortalama 2.3 gündü. Tüm hastalar sorunsuz taburcu edildi. Hastaların ortalama takip süresi 19 (dağılım 12-33 ay) aydı. Tüm olguların takip- leri sorunsuz seyretti. Sonuç: Apendiks karsinoid tümörleri sıklıkla apendektomi ile insidental saptanmaktadır. Kolorektal neoplazm gelişme olasılığı unutulmamalıdır.

Abstract

Background: In this study, we aimed to present the evalua- tion of patients with carcinoid tumors and the therapeutic ap- proach of appendiceal carcinoid tumors. Methods: Between January 2009 and December 2011, 1071 appendectomy cases were evaluated. Patients with histopath- ological examinations detecting carcinoid tumors were ana- lyzed retrospectively in terms of clinical signs, surgical signs, histopathology, and follow-up. Results: Six patients (0.6%) whose histopathological exami- nations detected carcinoid tumors were included in the study. Mean age was 28 (range 23 to 31 years). Five patients were male and one patient was female. It was not considered as tumoral formation during surgery in any case. All cases were evaluated as incidental carcinoid tumor. Mean tumor size was 0.5 cm (0.1-1.2 cm). Ki 67 was examined in 4 patients, and was under 2% in all 4 patients. Mean hospitalization time was 2.3 days. All patients were discharged without incidence. Mean follow-up time was 19 (range 12 to 33 months) months. The follow-ups were uneventful in all patients. Conclusion: Appendix carcinoid tumors are frequently found as incidental during appendectomy. The possibility of colorec- tal neoplasm should not be forgotten.
