Özet

Behçet hastalığı tekrarlayan oral ve genital ülserler, cilt lez- yonları ve üveit ile karakterize, etiyolojisi bilinmeyen multi- sistemik, enflamatuvar bir vaskülittir. Vena kava superiyortrombozu nadirdir ve olguların yalnızca %9.8ini oluşturur.Kırk iki yaşında erkek hasta hemoptizi şikayeti ile başvurdu.Fizik muayenede boyun ve üst ekstremitede şişlik ve göv- de ön duvarında yaygın dilate venöz kollateraller gözlendi.Ağzında aft saptanan hastanın oral ve genital aftlarının 17yıldır tekrarladığı öğrenildi. Skrotal bölgede ülserasyona aitskatris izlendi. Toraks bilgisayarlı tomografi anjiyografisindevena kava superiyor, subklaviyan ven ve azigos ven distalin- de trombüs saptandı. Bu bulgular ile Behçet hastalığı tanısıkonuldu. Olgumuzu nadir bir prezantasyon olması nedeniyle literatür bilgileri ışığında tartışmak amacıyla sunuyoruz.

Abstract

Behçet s disease is a multisystemic vasculitis of unknown eti- ology which is characterized by recurrent ulcers of the mouthand genitalia, cutaneous lesions, and uveitis. Superior venacava (SVC) thrombosis is rare and reported in 9.8% of patients.A 42-year-old male patient was admitted to our clinic with he- moptysis. Physical examination revealed swelling of the neckand upper ex tremities and dilated collateral veins on the uppertrunk. Oral aphthosis was observed, and the patient reportedoral/genitalia aphthosis for 17 years. Scarring of scrotal ulcerswere observed. The CT angiography of the chest demonstratedthrombosis of the SVC, subclavian veins, and azygos veins.Based on these findings, the patient was diagnosed with Be- hçet s disease. We present this rare case in order to discuss andcorrelate with the literature.
