Postoperatif vajinal kubbe sellülitinde etkili faktörler

Özet
Amaç: Vajinal kubbe sellülitinde etkili olabilecek faktörler prospektif olarak araştırıldı. Materyal ve Metod: Benign endikasyonlar nedeniyle histerektomi planlanan 43 olgu çalışmaya dahil edildi. Birinci kuşak sefalosporin ile profılaksi ve abdominal kesi yeri ve vajinal antisepsi sonrası, total abdominal histerektomi uygulanan ve vajinal kubbe kapalı olarak oluşturulan tüm hastalardan, preoperatif vajinal ve postoperatif kubbe kültürleri alındı. Operasyon süresi, intraoperatif kanama miktarı ve toplam hastanede kalış süresi kaydedildi, postoperatif febril ve enfeksiyöz morbidite takibi yapıldı. Bulgular: Hastaların yaş ortalaması 45.5 idi. Hastalardan %61'inin alafranga, %39'unun alaturka tuvalet kullandığı, %37.2'sinin tuvalet kağıdı ve %35 olgunun vajinal duş alışkanlığı olduğu öğrenildi. Menstruasyon döneminde %55 olgunun bez, %39 olgunun hijyenik ped ve %6 olgunun pamuk kullandığı saptandı. Postoperatif bir olguda abdominal kesi yeri enfeksiyonu ve bir olguda da vajinal kubbe hematomunu takiben enfeksiyon gözlendi. Preoperatif vajinal kültürlerde en sık izole edilen mikroorganizma Gardnerella vaginalis, sonra sırayla Candida ve E. Coli idi. Postoperatif vajinal kubbe kültürlerinde birinci sırada E. coli görüldü ve bu sonuçların tuvalet temizliğinde kağıt kullanmayan hasta grubuna ait olduğu saptandı. Preoperatif ve postoperatif kültürler karşılaştırıldığında sadece 4 olguda (%9.3) aynı patojenin postoperatif dönemde persiste ettiği gözlendi. Sonuç: Postoperatif morbiditenin önlenmesinde, titiz bir cerrahi teknik ve antibiyotik profilaksisi yanında, vajinal antisepsinin önemi büyüktür.
Abstract
Objective: To evaluate prospectively the factors which affect the postoperative vaginal cuff cellulitis. Material and Methods: Forty-three women scheduled for hysterectomy for various benign indications were included in the study. In all cases, after prophylaxis with a first generation cephalosporin and abdominal and vaginal scrubbing in the operating room, total abdominal hysterectomy was performed with closed vaginal vault. Preoperative vaginal and postoperative cuff cultures were obtained. The operating time, the intraoperative blood loss and the hospitalization time were recorded; the postoperative febrile and infectious morbidity were followed. Results: The median age was 45.5 years. Sixty-one percent of the cases were using Western style toilet, 39% were using Turkish style toilet; 37.2% were using toilet paper and %35 have the habit of vaginal washing. In the menstruation period, 55% of the patients were using diaper, 39% were using hygienic pad and 6% were using cotton. Only one abdominal incision and one vaginal cuff infection following vaginal vault hematoma occurred. Preoperatively the most commonly isolated microorganisms were Gardnerella vaginalis, Candida albicans and Escherichia Coli. E. coli was the most frequent isolated microorganism in the postoperative vaginal cuff cultures and these results were those of the patients who were not using toilet paper. Comparing preoperative and postoperative cultures, it is found that only in four patients (9.3%), the same microorganism persisted. Conclusion: To prevent postoperative morbidity, beside a meticulous surgical technic and antibiotic prophylaxis, vaginal antisepsis was important.
