Özet

Fetal ağırlığın 4500 gramın üzerinde olduğu doğumlarda, doğum şekli ile ilgili perinatal ve maternal risklerin belirlenmesi. Mateyal ve Metod: Şişli Etfal Eğitim ve Araştırma Hastanesi, 1 Kadın Hastalıkları ve Doğum Kliniğinde, Kasım 1992-Kasım 2002 tarihleri arasında, doğum tartısı 4500 gramın üzerinde olan 293 doğumdan 75'i abdominal yolla (sezaryen) olmuştur (Grup I). Kalan 123 doğum vaginal yolla gerçekleşmiş olup, 75 tanesi randomize edilerek çalışmaya alınmıştır (Grup II). Her iki grupta perinatal ve maternal komplikasyonlarretrospektif olarak incelenmiş ve karşılaştırmıştır. Bulgular: Vaginal doğumda; omuz distosisi (%14.6) ve annede genital travma (%28) en sık görülen komplikasyonlardı. Sezaryende; omuz distosisi ve genital travma yoktu. Diğer perinatal ve maternal komplikasyonlar bakımından, iki grup arasında istatistiksel anlamlı fark bulunmamıştır(p>0.05). Sonuçlar: Fetal ağırlığın 4500 gramın üzerinde olduğu doğumlarda, omuz distosisi ve maternal genital travma, vaginal doğum ile ilgili komplikasyonlar olarak bulunmuştur. Ancak, bu komplikasyonlar makrozomik vaginal doğuma özgü olmadığından, sezaryen vaginal doğumdan üstün bulunmamıştır.

Abstract

Objective: To determine perinatal and maternal risks in relation with the mode of delivery as for the fetuses weighing above 4500 grams". Materials and Methods: In Şişli Etfal Training and Research Hospital, $1^{st}$ Gynecology and Obstetrics Clinics there were 293 deliveries in which the birth weight was above 4500 grams between November 1992-November 2002. Cesarean section was performed for 75 of these (Group I). Rest 123 women were delivered vaginally and, randomized 75 of these were included in the study (Group II). Perinatal and maternal complications were compared between the groups. Results: Shoulder dystocia (14.6 %) and maternal genital trauma (28 %) were the most common complications in vaginal deliveries. There were not any shoulder dystocia and genital trauma detected in cesarean deliveries. No statistically significant difference was found between the groups in respect of other perinatal and maternal complications (p>0.05). Conclusions: Shoulder dystocia and maternal genital trauma were the complications related with the mode of delivery for the births in which fetal weight is above 4500 grams. However, these complications are not specific to macrosomic vaginal deliveries. For that reason, these were not considerable complications in relation with the mode of delivery. Cesarean section wasn't found superior to vaginal delivery for the births in that fetal weight was above 4500 grams.
