Özet

Amaç: Polikistikoversendromlu(PKOS) olgularda lipid profilinin değerlendirilmesi. Materyal ve Mctod: Tek merkezli, vaka kontrol niteliğindeki çalışmamıza, Kasım 2003-Mart 2004 tarihleri arasında, infertilite yakınması olan ve PKOS tanısı alan 18 kadın ile (çalışma grubu) erkek faktör infertilitesi olan ve herhangi bir medikal problemi olmayan 10 kadın (kontrol grubu) dahil edildi. Tüm olgularda menstruasyonun 3. günü, bir gecelik açlığı takiben hormon ve lipid profili değerlendirildi. Bulgular Student's t test ve Mann-Whitney U test ile değerlendirildi. Bulgular: Her iki grup; yaş, vücut kitle indeksi ve bel/kalça oranı açısından benzerdi. LH, total testosteron, androstenedione düzeyleri ve Ferriman-Gallwey hirşutismus skoru PKOS grubunda anlamlı oranda yüksekti. Estradiol, DHEA-S, total kolesterol (TC) ve düşük dansiteli lipoprotein (LDL-C) seviyeleri her iki grupta benzerdi. Ortalama trigliserid (TG) düzeyi PKOS grubunda anlamlı oranda yüksek olarak saptanırken (sırası ile; 114.2±38,8 mg/dL, 81.1±29.1 mg/dL; p= 0.049), yüksek dansiteli lipoprotein (HDL-C) düzeyi PKOS grubunda anlamlı oranda düşük olarak saptandı (sırası ile; 48.0±5.9mg/dL, 66.2±12.3 mg/dL; p=0.001). Çok düşük dansiteli lipoprotein (VLDL-C) düzeyi PKOS grubunda kontrol grubuna oranla anlamlı oranda yüksek saptandı (sırası ile; 22.8±7.8 mg/dL, 16.2±5.8 mg/dL; p=0.049). Sonuç: Dislipidemi uzun dönemde gelişebilecek olan kardiyovasküler hastalık riskinde artış ile beraberdir. PKOS'lu kadınlarda hiperandrojenizm vücut kitle indeksi ve vücut yağ dağılımından bağımsız olarak lipid metabolizmasını kötü yönde etkileyen bir faktördür.

Abstract

Aim: In this study we aimed to study lipid profile in patients with polycystic ovarian syndrome (PCOS). Material and Method: This was a case control study done in a single center between the dates October 2003 and March 2004. Eighteen women who presented with infertility and diagnosed with PCOS were included in this study as the study group, and ten women with infertility due to male factor and without any medical problems were included as the control group. Samples were drawn from all the cases on the third day of the menstrual cycle after an overnight fasting for lipid and hormone profile analysis. Student's t test and Mann-Whitney U test were used for statistical analysis. Results: Both groups were similar in age, body mass index and waist/hip ratio parameters. Luteinizing hormone (LH), total testosterone, androstenedione levels and Ferriman-Gallwey hirsutism score were significantly higher in the PCOS group. Estradiol and dihydroepiandrosterone sulphate (DHEA-S), LDL and total cholesterol levels were similar in each group. Triglyceride levels were higher in the PCOS group when compared with the control group (114.2±38.8 mg/dL , 81.Ü29.1 mg/dL respectively; p=0.049), whereas HDL levels were lower in the PCOS group (48.0±5.9mg/dL, 66.2±12.3 mg/dL respectively; p=0.001). VLDL levels in the PCOS group were significantly higher than the control group (22.8±7.8 mg/dL, 16.2±5.8 mg/dL; p=0.049 respectively). Conclusion: There is a strong correlation between dyslipidemia and cardiovascular disease in the long term. We believe hyperandrogenism, encountered in patients with polycystic ovarian syndrome, negatively impacts lipid metabolism regardless of the body mass index and body fat distribution.
