Özet
Amaç: Fertilitenin ve dolayısıyla uterusun korunması istenen uterin prolapsuslu genç kadınlarda abdominal sakrohisteropeksi cerrahisinin başarı ile uygulanabilirliğini göstermektir. Çalışmanın Yapıldığı Yer: TCSB Ankara Etlik Doğumevi ve Kadın Hastalıkları Eğitim Araştırma Hastanesi. Olgu Sunumu: 26 yaşında, total uterin prolapsus tanısı ile ilk olarak Baldy-Webster + sakrouterin plikasyon + modifiye Moskowitch cerrahisi uygulanmış hastanın postoperatif 8. ayda nüks uterin prolapsus gelişmesi üzerine sentetik mersilen mesh kullanılarak abdominal sakrohisteropeksi uygulandı. Olguda erken dönemde intraoperatif veya postoperatif komplikasyon gelişmedi. Hastanın 3 yıllık izlemi sonrasında I. derece sistosel dışında nüks uterin prolapsus, feçeste topaklanma, konstipasyon,koital sorun veya idrar inkontinensi gözlenmedi. Sonuç: Bu teknik genç kadınlardaki uterin prolapsusun düzeltilmesinde kolay sayılabilecek ve efektif bir metoddur. Mersilen mesh ise uyumlu, güçlü, kolay şekil verilebilen bir askı maddesidir. Olgumuzdaki gibi daha önce uterin prolapsusu nedeni ile başka operasyonlar geçirmiş nüks olgularda denenebilir.
Abstract
Objective: To show that, when fertility is major concern in young women with uterine prolapsus, abdominal sacrohysteropexy will be applied successfully for conservation of uterus. Institution: Ministry of Hearth, Ankara Etlik Maternity and Women's Health Teaching and Research Hospital, Turkey. Case: A 26 years old patient with recurrent uterine prolapsus, who had been operated 8 months ago with the diagnosis of total uterine prolapsus and Baldy-Webster plus sacrouterine plication plus modified Moschowitz operation done, was reoperated, and sacrohysteropexy by using synthetic Mersilene mesh was done. No complications were seen intraoperatively or during the early post-operative period. During 3 years follow-up period, no recurrent uterine prolapsus, constipation, coital problems, urinary incontinence, except 1 st degree cystocele were recorded. Conclusion: This technique is an easy and effective method for repair of uterine prolapsus in young women. Mersilene mesh is a strong, easily reformable, and non-reactive synthetic material for sling operations. It can be used effectively for patients who had surgery previously for uterine prolapsus, but recurrence occured.
