Özet

Amaç: Çoğul gebelik redüksiyonu yapılan olguların gebelik sonuçları açısından değerlendirilmesi. Materyal ve Metot: Bu çalışmada klini¤imizde yardımcı üreme teknikleri ile elde edilmiş ve çoğul gebelik redüksiyonu yapılan 119 olgunun geriye dönük olarak dosyaları değerlendirilmiştir. Redüksiyon öncesi/sonrası fetus sayısı, abortus olup olmadığı, doğum haftası, doğum kilosu, gebelik komplikasyonları ve demografik özellikler dosya bilgilerinden elde edilmiştir. İndirgenme sonrası kalan fetus sayısına göre düşükler, preterm eylem ve perinatal sonuçlar karşılaştırılmıştır. Tüm olgularda çoğul gebelik redüksiyonu işlemi; 10-14 hafta arasında, ultrasonografi eşliğinde 22-gauge iğne ile fetal toraksa girilerek, gebelik haftasına bağlı olarak 2-3 ml %10’luk potasyum klorid solusyonunun fetal kalbe enjekte edilmesi ile yapılmıştır. Sonuçlar: Tüm grupta preterm doğum ve perinatal mortalite oranı sırası ile 87/108 (%80.55) ve 6/108 (Binde 55.6) olarak bulunmuştur. Ortalama abortus haftası 16.09±2.91 iken ortalama doğum haftası 34.08±3.03 hafta olarak saptanmıştır. Araştırma grubunda olguların 86’sında (%72.30) 3’den 2’ye, 31’inde (%26.10) 4’den 2’ye, 2 olguda (%1.70) ise 3’den 1’e redüksiyon yapılmıştır. Her üç gruptaki fetal kayıp oranı sıra ile %6.97 (6 olgu), %16.12 (5 olgu) ve %0.00 olarak bulunmuştur (p>0.05). Redüksiyon başlangıç fetus sayısı ile abortus, preterm doğum, perinatal mortalite oranları, doğum haftası, doğum ağırlığı arasında istatistiksel anlamlı fark saptanamamıştır. Buna karşın başlangıçta fetus sayısı çok olan grupta, az olan gruba göre, ortalama abortus haftası istatistiksel anlamlı olarak daha yüksek bulunmuştur. Tartışma: Çoğul gebelik redüksiyonu; eve en azından bir bebek götürme oranının %85.71 olarak saptandığı bu çalışmada da gösterildiği üzere; güvenli bir seçenek olarak kullanılabilir. Çoğul gebelik redüksiyonu üçüz ve dördüz gebeliklerin neden olduğu gebelik kaybı ve ciddi prematürite risklerini azaltabilir.

Abstract

Objective To report the pregnancy outcome of multifetal pregnancy reduction (MFPR). Methods This was a retrospective study evaluating the outcome of 119 pregnancies after multifetal pregnancy reduction. The data, including numbers of fetuses before and after fetal reduction, whether abortion has occurred or not, birth weight, gestational week at delivery, pregnancy complications, and demographic characteristics were obtained from clinic records and patients files. The comparison of preterm delivery and abortion rates and perinatal outcomes with regard to remaining number of fetuses following fetal reduction was made. In all of them pregnancy reduction was performed between 10-14 weeks under ultrasonography guidance. A 22-gauge needle was introduced into the thorax of the fetus and 2-3 ml of 10% potassium chloride solution was injected into the fetal heart, depending on the gestational age. Results The rates of preterm delivery, perinatal mortality were 87/108 (80.55%), and 6/108 (55.6‰), respectively. The mean age at abortion and delivery were 16.09 ± 2.91 and 34.08 ± 3.03 weeks, respectively. The 72.3% (n=86) of the patients were subjected to multifetal pregnancy reduction from 3 to 2, 31 (26.1%) of patients from 4 to 2, while 2 (1.70%) of them were subjected to MFPR from 3 to 1. The abortion rates in there groups were 6.9% (6 patients), 16.1% (5 patients) and nil respectively. There was no significant correlation between starting number of fetuses and miscarriage, preterm delivery, perinatal mortality rates, delivery age, and mean birth weight. In contrast, the high starting number of fetuses had significantly higher mean miscarriage age. Conclusion MFPR resulted in at least one live neonate in 85.71% of the cases and was effective in reducing the risks of pregnancy loss and severe prematurity in quadruplets and triplet pregnancies.
