Prenatal tanıda belirlenen ailesel resiprokal translokasyonunun (1q23;19q13.3) kırık bölgelerinin malignensilerle ilişkisi ve genetik danışma

Özet
Dengeli kromozomal yeni düzenlenimlerden olan resiprokal translokasyonlar, mayoz bölünmelerde normal gametlerin yanı sıra kromozomal olarak dengesiz gametler de oluşturabildikleri için, bunlarda verilen genetik danışma son derece zordur. Bu çalışmada, prenatal olarak 1. kromozom (q239) ile 19. kromozom (q13.3) arasında saptanan resiprokal translokasyonun tanımlanmasında izlenen yol tartışıllmaktadır. ileri anne yaşı nedeniyle 17. gebelik haftasında amniyosentez girişimi uygulanmıştır. Bunu takiben translokasyonun ailesel veya de novo olup olmadığını belirlemek için ilk aşamada, aralarında akrabalık olmayan ebeveynlerde sitogenetik çalışma yapılmıştır. Aynı kromozom kuruluşu, fenotip olarak sağlıklı annede de belirlendiği için, bu yapısal değişim ailesel olarak değerlendirilmiştir. Annenin kendi ebeveynlerinin ve kızının da aynı yapısal değişime sahip olduğu görülmüştür. Akut lenfoblastik lösemide, 1. kromozomun q23 bölgesindeki kırık, 19p13 ile birleşerek şimerik gen oluşturduğu için ayrıca önem taşımaktadır. Bu bilgiyi edinen aile anksiyete yaflamış ve gebeliğin olası prognozu hakkında kararsız kalmıştır. Sunulan olguda belirlenen translokasyonun, sadece bir bölgesi (1q23) şimerik gen ile homoloji gösterdiği için, aileye buradaki olası anomali riskinin (%5-10) genel ailesel kromozomal yeni düzenlenimlerden farklı olmadığı söylenmiştir. Bu bilgi doğrultusunda aile gebeliğe devam kararı almıştır.
Abstract
Genetic counselling of reciprocal translocations is quite difficult, as these balanced chromosomal rearrangements may produce both balanced and unbalanced gamets at meiosis. In this study, the proposed strategy for the identification of a prenatally determined balanced reciprocal translocation involving chromosomes 1q23 and 19q13.3 was discussed. Amniocentesis was performed because of advanced maternal age at the 17th weeks’ of gestation. First, the non-consanguineous parents who have a healthy living child, were offered to have their own karyotypes to determine the present rearrangement as familial or de novo. As we came across with the same translocation in the healthy mother, it was addressed as familial. The mother and the sister of her also had the same chromosomal rearrangement. The breakpoint of long arm of chromosome 1q23 was important as the fusion of this region with 19p13 could produce a new chimeric gene in acute lymphoblastic leukemia. The parents were anxious about this information and the probable prognosis of pregnancy confused them. Based on the knowledge of only the one breakpoint region (1q23) share the homology with the chimeric gene formation, the parents decided to give birth to their fetus who had the general, 5-10%, risk of abnormalitity related to balanced familial rearrangements.
