Management of Spinal Epidural Lipomatosis: A Report of Two Cases

Özet
Spinal epidural Lipomatozis (SEL), omurilik veya dural kesede basıya neden olan nadir birhastalıktır. Erkeklerde daha sık görülmekle beraber çoğunlukla torasik omurgayıetkilemektedir. Bu durum genellikle aşırı endojen steroid üretimi veya sistemik bir rahatsızlıknedeni ile alınan aşırı dozda eksojen steroid kullanımı sonucu ortaya çıkabilmektedir. SELbenign lezyondur, sıklıkla bel ağrısı veya nörojenik kladikasyo ile ortaya çıkar, bel ağrısı veradikulopatisi olan hastalarda ayırıcı tanıda düşünülmelidir. Nörolojik defisit yoksa obezite,steroid kullanımı ve endojen steroid hastalıklarında konservatif tedavi yapılmalıdır. SEL'decerrahi tedavi, inatçı ağrı veya nörolojik defisiti olan hastalarda altın standarttır.
Abstract
Spinal epidural lipomatosis (SEL) is a rare cause of spinal cord or dural sac compression. It ismore common in males and usually affects the thoracic spine. This condition is often aconsequence of excessive endogenous corticosteroid production or exogenous steroidadministration for various intercurrent systemic diseases. The case histories of two patientssuffering from lumbar SEL are reviewed and the management of the patients discussed, weare encouraged to report two cases. They are benign lesions, frequently presenting low backpain or neurogenic claudication, should be considered in the differential diagnosis, in patientswith low back pain and radiculopathy. Conservative treatment should be attempted withobesity, steroid use and endogenous steroid disease, provided that there are no neurologicaldeficits. Surgical treatment with SEL is the gold standard in patients with intractable pain orneurological deficit.
