Percutaneous Biopsy of the Spine: Analysis of 84 Cases

Özet
Amaç: Omurgadan perkütan biyopsi lokal anestezi altında, ayaktan hastalara uygulanabilir.Patolojik ya da mikrobiyolojik tanı sonucuna göre, hastalara cerrahi girişim gerekmedenradyoterapi veya antibiyoterapi ile tedavi uygulanabilir. Bu çalışmada, perkütan omurgabiopsisi ile ilgili deneyimlerimizi sunmayı amaçladık.Yöntem ve Gereç: Kliniğimizde 1995-2007 yılları arasında, radyolojik incelemelerindespinal patoloji saptanarak perkütan biyopsi uygulanmış 84 hastanın dosyası retrospektif olarakincelenmiştir.Bulgular: 49 erkek, 35 kadın olmak üzere toplam 84 hastaya çeşitli nedenlerle omurgabiyopsisi uygulandı. Biyopsilerin, 5'i servikal, 18'i torakal, 49'u lomber ve 12'si sakral bölgedeyapıldı. Biyopsi sonuçları 32 olguda spondilodiskit, 17 olguda primer tümör ve 24 olgudametastatik tümör tanısı aldı. 11 biyopsi sonucu ise tanı koydurucu olmadı. Spondilodiskittanılı hastaların biyopsi materyallerinde en sık izole edilen etken Stapylococcus Aureus, ensık 2. etken ise Mycobacterium Tuberculosis olmuştur. Serimizde enfeksiyon en sık lomberbölgede yerleşmiştir (32 spondilodiskitin 19' u), en sık metastaz da yine lomber bölgede ( 24metestazın 13' ü) saptandı. Biyopsi sonucuna göre toplam 34 hastaya cerrahi girişimuygulandı. Serimizde, uygulanan girişim ile ilgili bir komplikasyon ya da mortalite ilekarşılaşılmamıştır. Biyopsi olgularımızın %87'si tanı koydurucu olmuştur.Sonuç: Perkütan omurga biyopsisi, omurganın patolojik lezyonlarının kesin tanı ve tedavisiiçin önemli bir tanı yöntemidir. Doğruluğu ve düşük komplikasyon oranı BilgisayarlıTomografi eşliğinde yapılan biopsi tekniği ile benzerdir.
Abstract
Objective: Percutaneous vertebral biopsy can be performed under local anesthesia on anoutpatient basis. So then additional therapies such as radiotherapy, chemotherapy andantibiotherapy may be planned without an open surgery after getting histopathological ormicrobiological results. This study aims to analyse the accuracy and results of a series ofpatients that we performed percutaneous biopsy of the spine.Methods: The medical records of 84 patients with vertebral pathologic lesions whounderwent percutaneous vertebral biopsy at our institution between 1995 and 2007 werereviewed.Results: There were 84 patients (49 male, 35 female). The biopsies were performed in thefollowing regions: cervical (5), thoracic (18), lumbar (49) and sacral (12). The diagnoses werespondylodiscitis in 32, primary tumors in 17, and metastatic tumors in 24 cases. Elevenbiopsies were not diagnostic. In patients with spondylodiscitis, the most common isolatedmicroorganism was Staphylococcus aureus (13), followed by Mycobacterium tuberculosis (9).The lumbar spine was the most common location of infection (19 of 32) and also of metastatictumors (13 of 24). 34 patients underwent a surgical procedure according to the biopsy results.There were no post procedure complications or mortality in our series. 73 of our biopsies(87%) were diagnostic.Conclusions: Fluoroscopic guided percutaneous vertebral biopsy is still an importantdiagnostic method for the accurate diagnosis and treatment of vertebral pathologic lesions. Itsaccuracy and low complication rate are similar to that of the CT guided biopsy technique.
