Neurologic signs due to leptomeningeal ınvolvement as the presenting manifestation of gastric adenocarcinoma

Özet

Beş aydır olan ilerleyici nörolojik bulgular nedeniyle hastanemize başvuran 43 yaşındaki kadın hastanın muayenesinde multipl kranial sinir tutulumu ve solda serebellar sendrom bulguları saptanmıştır. BOS analizinde belirgin protein yüksekliği, serebral MRG’de ise leptomeningeal ve serebellar kontrast tutulumu gösteren lezyonlar tespit edilmiştir. Son semptomların ortaya çıkışına dek belirgin gastrointestinal sistem yakınmaları olmamasına karşın olgunun etiyolojik araştırma sırasında yapılan panendoskopi ve sitolojik değerlendirmesinde gastrik adenokarsinom patolojik tanısı konulmuştur. Olgumuz, gastrik adenokarsinomda leptomeningeal tutuluma bağlı olarak, kinik tablonun nörolojik bulgularla ortaya çıkması nedeniyle sunulmaya değer bulunmuştur.

Abstract

A 43-year-old female with a 5-month history of progressive neurological manifestations was admitted to our hospital. On examination, she was found to have multiple cranial nerve deficits and cerebellar syndrome. Laboratory investigations revealed a prominent increase in protein content with no significant pleocytosis in cerebro spinal fluid. Neuroimaging studies showed contrast enhancement on cerebellar cortex and leptomeningeal enhancement at brainstem. Although she had no complaints regarding gastrointestinal system, further investigatons led to the diagnosis of gastric adenocarsinoma. We report this case for the rare presentation of gastric adenocarcinoma with neurological manifestations due to leptomeningeal involvement.
