Ototoxicity probably due to isoniazid in a patient undergoing hemodialysis

Özet

Son dönem böbrek yetmezliği olan olgularda tuberküloz insidansı yüksektir. Bu grup hastalarda antitüberküloz ilacın indüklediği ototoksisite bir problem olarak karşımıza çıkabilir. Streptomisin, ototoksik yan etkileri iyi bilinen bir antitüberküloz ilacıdır. Ancak, hemodiyalize giren 42 son dönem böbrek yetmezliği olan olgunun 7'sinde kullanılan ilaç kombinasyonunda streptomisin olmadığı halde ototoksisiste bildirilmiş ve bu durumdan nörotoksik yan etkileri iyi bilinen başlıca izoniazidin sorumlu olabileceği ileri sürülmüştür. Bu bildiride, strepromisini içermeyen, ancak izoniazidin dahil olduğu antitüberküloz ilaç kombinasyonu alırken işitme kaybı gelişen, hemodiyalize giren 78 yaşında erkek hasta sunulmuş ve izoniazidin ototoksisitedeki olası rolü tartışılmıştır.

Abstract

Patients with end-stage renal failure (ESRF) have a high incidence of tuberculosis. Antituberculosis drug-induced ototoxicity may sometimes be a problem in these group of patients. Streptomycin is an antituberculosis drug with well-known ototoxic side effects. However, ototoxicity with combinations not including streptomycin has been reported only in a report in 7 of 42 ESRF patients undergoing hemodialysis and isoniazid has been suggested as a probable responsible drug which has well-known neurotoxic side effects. Here, we presented a 78- year-old man developed hearing loss who was on hemodialysis program and taking antituberculosis drug combination therapy including not streptomycin, but isoniazid, and the probable role of isoniazid in ototoxicity was discussed.
