The relationship between physical performance, cognition and depression in alzheimer type of dementia

Özet

Alzheimer hastalığı progresif bir rahatsızlık olup kognitif ve fonksiyonel kapasitede azalmaya yol açar. Sağlıklı yaşlılarda motor fonksiyon ile kognitif bozukluk arasındaki ilişkiyi gösteren çok sayıda çalışma mevcuttur. Ayrıca depresyon ve kognitif durum arasındaki ilişki de daha önce tartışılmıştır. Bu çalışmadaki hipotezimiz fiziksel performansın kognitif yetersizliklere bağlı olduğudur ve bu nedenle kognisyon, fiziksel durum ve depresyon arasındaki ilişkiyi incelemeyi amaçladık. Çalışmaya Dokuz Eylül Üniversitesi Demans polikliniğinde kayıtlı NINCDS-ADRDA kriterlerine göre Alzheimer hastalığı tanısı almış 26 (21 kadın, 5 erkek) hasta dahil edilmiştir. Hastalarda fiziksel, depresif ve kognitif durumu değerlendirmek için Fiziksel Performans Testi (FPT), Mini Mental Durum Değerlendirmesi (MMDD), Stroop testi, Yesavage Geriatrik Depresyon Skalası (YGDS) ve Enstrümental Günlük Yaşam Aktiviteleri Skalası kullanılmıştır. FPT ile kognitif testler [(Stroop testi; renkli kelimelerin rengini ayırt etme süresi (r: 0.5152 p: 0.012)] ve MMDM [(r: 0.5564 p: 0.006)] ve YGDS (r:-0.4304 p: 0.040) arasında yaş, cinsiyet ve eğitim süresine göre kontrollü analiz yapıldığında anlamlı korelasyonlar saptanmıştır. Çalışmanın sonucunda Stroop testinin depresyon ve fiziksel fonksiyonlar arasındaki ilişkiyi yansıtmada daha hassas bir araç olduğu saptanmıştır. Ayrıca fiziksel ve kognitif yetersizliklere yönelik olarak bütünleştirilmiş tedavi yaklaşımları dizayn edilmesi gerektiğine inanmaktayız.

Abstract

Alzheimer disease is a progressive disorder and leading to decrease in cognition, and functional capacity. There are several reports on the relationship between motor function and cognitive impairment in healthy elderly. And also the relationship between depressive and cognitive status has been discussed before. We hypothesized that physical performance is related to cognitive deficits and aimed to examine the relationship between cognition, physical status and depression in this study. The study consisted of 26 patients with probable Alzheimer type of dementia (21 female, 5 male) according to NINCDS-ADRDA criteria, who were admitted to Dokuz Eylul University Dementia outpatient clinic. The Physical Performance Test (PPT), the Mini-Mental State Examination (MMSE), the Stroop Test, the Yesavage Geriatric Depression Scale (YGDS), and the Instrumental Activities of Daily Living Scale (IADL) were used to evaluate the physical, depressive and cognitive status.
