Spinal Görüntüleme Bulgusu Olan ve Subakut Kombine Dejenerasyon Tanısı Alan Sekiz Hastanın Klinik, Demografik ve Radyolojik Özellikleri

Özet

Bilimsel Zemin: Vitamin B12 eksikliğinde beyin, optik sinir, periferik sinir ve spinal kordunetkilenmesine bağlı olarak değişik nörolojik tablolar görülebilir. Vitamin B12 eksikliğindespinal kordun etkilenmesi sonucu ortaya çıkan subakut kombine dejenerasyon (SKD), erkentanı ve tedavi edildiğinde geri dönüşlü olabilen bir tablodur. Biz burada, klinik semptomlarıile korele olarak, manyetik rezonans görüntülemelerinde (MRG) spinal kordun posterior velateral kolumnalarında değişiklikler görülen sekiz SKD olgusu sunuyoruz.Hastalar ve Metod: Demiyelinizan hastalık ayırıcı tanısı için Multipl skleroz polikliniğindedeğerlendirilen, spinal MRG'de patolojik görüntüleme bulgusu olup SKD tanısı alan sekizhastasının klinik, demografik ve radyolojik özellikleri belirlendi.Sonuçlar: SKD tanısı alan hastaların 5'i erkek, 3'ü kadındı. Spinal görüntülemelerde 7hastada servikal, 1 hastada torakal bölgede miyeliti düşündüren radyoljik görünüm vardı.Hastaların tümüne BOS incelemesi yapıldı ve normaldi. Beyin MRG hastaların tümündenormaldi. Hastaların tümünde serum vitamin B12 düzeyi 150 pg/ml'nin altındaydı vehiçbirinin hematolojik incelemesinde anemi yoktu. Subakut başlangıç olan 6 olguda vitaminB12 replasmanı ile tedavinin ilk altı ayı içerisinde tama yakın düzelme, kronik öyküsü olan 2olguda ise tedaviden sonraki 1 yıl içerisinde kısmi düzelme görüldü.Yorum: B12 vitamini yetersizliğinin majör hematolojik belirtisi olan megaloblastik anemiolmaksızın hastaların bir kısmında nörolojik belirti ve bulgular görülebilir. En önemli bulguspinal MRG'de kordun posterior kolonlarında T2'deki sinyal değişikliğidir. Bununla birlikteMRG özellikleri SKD için spesifik değildir. Hastalarda klinik ve laboratuar özellikleri ilebirlikte diğer ayırıcı tanıların da yapılması gereklidir.

Abstract

Scientific Backgrounds: The different neurologic manifestations of vitamin B12 deficiencyare the results of its effects on the brain, optic nerves, peripheral nerves and spinal cord.Subacute combined degeneration (SCD) of the spinal cord is a complication of vitamin B12deficiency, which is reversible if diagnosed and treated early. We report eight cases of SCDwhose magnetic resonance imaging (MRI) of the spinal cord showed changes in the posteriorand lateral columns, correlating well with patients clinical symptoms.Patients and Methods: The clinical, demographical and radiological characteristics of eightpatients who had pathological imaging findings in spinal MRI with the diagnosis of SCD andassessed by the clinic of multiple sclerosis for the differential diagnosis of demyelinatingdisease were determined.Results: Patients who diagnosed SCD, 5 were male and 3 were female. In spinal imagings,was suggestive that the radiological appearance of servical region myelitis in seven case andthoracic region myelitis in one case. Cerebrospinal fluid (CSF) examination was performedand it was normal in all patients. Brain MRI of all patients were normal. In all patients, serumvitamin B12 level was lower than 150 pg/ml and nobody had anemia in hematologicalexamination. Nearly full recovery was seen in six patients who have subacute onset andtreated with vitamin B12 replacement in the first six months of therapy and also partialrecovery was seen in two patients who have a history of chronic course, with in one year afterthe treatment.Comments: Some patients who show neurological symptoms and signs may not havemegaloblastic anemia which is the major hemotological complication of vitamin B12deficiency. The most significant sign of the disease may be seen in spinal MRI whereposterior columns of the cord show signal changes in T2 weigheted series. However, MRIfeatures are not specific for SCD. The other differential diagnosis with clinical and laboratoryfeatures need to be done in patients.
