Primary Gliosarcoma: Clinical and Pathological Evaluation of Four Cases

Özet
Amaç: Gliosarkoma, glial ve mezenkimal diferansiasyon alanları içeren bifazik dokupaternine sahip, glioblastomun bir varyantıdır. Santral sinir sistemi tümörlerinin nadir görülenbir türüdür. Gliosarkomlar, glioblastomların %2'sini oluşturmaktadır. Tümör prognozaçısından glioblastomaya benzerlik göstermektedir. Kliniğimizde glisarkom tanısı alan dörtvakanın klinik ve histopatolojik özellikleri tartışılmıştır.Method: Nörolojik muayene ve görüntüleme yöntemleri ile tümör tanısı kondu. Hastalardanüçü erkek biri kadın idi. Yaş aralığı 33-66 arasındaydı. Tümörlerin hepsi supratentorialyerleşimliydi. Tüm hastalara kraniotomi ile cerrahi uygulandı. Cerrahi sonrası üç hastayaradyoterapi verildi.Bulgular: Cerrahi rezeksiyon ve radyoterapiye rağmen bir hastamız erken dönemde öldü. 9- 13 aylık takipler sonrasında tümörlerin nüks ettiği görüldü. Tüm hastalarımız bu takipsüresinde kaybedildi. Histopatolojik olarak; gliosarkom için tipik olan gliomatöz vesarkomatöz alanlar içeren bifazik doku paterni tespit edildi.Sonuç: Gliosarkoma klinik olarak agresif seyirli olup oldukça kötü bir prognoza sahiptir. Butümörler WHO (World Health Organization) sınıflamasına göre grade IV kabul edilip, nüksoranı yüksektir. Tüm kötü prognozlu glial tümörler gibi cerrahi rezeksiyon ve radyoterapi eniyi tedavi yöntemidir.
Abstract
Aims and Background: Gliosarcoma is an uncommon primary malignant tumor of thecentral nervous system. Gliosarcoma is a variant of glioblastoma, which includes glial andmesenchymal differentiation sites and has biphasic tissue patterns. They form up to 2% of allglioblastomas, and are similar to the glioblastoma in terms of prognosis. Four patientspresenting with gliosarcoma were reviewed and are discussed herein with respect to theirclinical and histopathological characteristics. The diagnosis was suspected based onradiological findings and was confirmed by histological examination.Methods: The tumors were diagnosed with neurological examination and neuroimagingmethods. There were 4 patients (3 males and 1 female; age range: 33 to 36 years). All thetumors were located in the supratentorial region. All the patients underwent surgery withcraniotomy and three of them received post-surgical radiotherapy (RT).Results: Despite the surgical resection and RT, one patient died in the early stage.Recurrences were seen in the follow-up exams between 9 13 months. All of the patients diedin the follow-up period. Pathological diagnosis of the patients was gliosarcoma. Biphasictissue pattern including gliomatosis and sarcomatosis sites, which is a typical characteristic ofgliosarcoma, was histopathologically detected.Conclusions: Gliosarcoma is also seen clinically with aggressive course and poor prognosis.These tumors are accepted as grade IV according to the World Health Organizationclassification and the recurrence rates are high. Surgical resection and RT are the mostefficient treatment procedures as in all glial tumors, with poor prognosis.
