Interhemispheric Subdural Hematoma in An Adult: Case Report

Özet
Erişkinlerde interhemisferik subdural hematom, subdural hematomun nadir rastlanan birtipidir. Paryetal-oksipital korteks ve superior sagital sinus arasındaki venlerin yırtılmasısonucunda oluşur. Bilgisayarlı tomografide, falks altında sagital sinüsten ayrılan hafifkalınlaşmış bir şerit görülür. Biz burada trafik kazası nedeniyle hastanemize getirilen 74yaşında bir bayan olguyu sunduk. Başvuru sırasında hastanın Glasgow Koma Skalası 11 idi(sözel uyaranla gözünü açıyor, ağrıyı lokalize ediyor, uygunsuz kelimeler söylüyor).Bilgisayarlı beyin tomografisinde oksipital bölgede intraserebral hematom ile birlikte birinterhemisferik subdural hematom görüldü. Hastaya konservatif tedavi uygulandı. Kazadan 4hafta sonra çekilen bilgisayarlı tomografide interhemisferik hematomda kısmi ve intraserebralkanamada tam emilim olduğu görüldü. 1 yıl sonra çekilen kontrol tomografi normaldi.İnterhemisferik hematom nadir görülen bir kanamadır. Durumu stabil olan ve iyileşmegösteren hastalarda sıkı klinik gözlem ile ilaç tedavisi uygulanabilir.
Abstract
Interhemispheric subdural hematoma in adults is a rare type of subdural hematoma. It iscaused by laceration of veins between the parietal-occipital cortex and superior sagittal sinus.Computed tomography shows a band of increased attenuation along the falx and remote fromthe sagittal sinus.We report a 74 year old woman admitted to the hospital with a history of a car accident. Inadmission her Glasgow Coma Scale was 11 (opening her eyes with verbal stimuli, localizes topain, verbalizing inappropriately). The cranial computerized tomography revealed aninterhemispheric subdural hematoma with intracerebral hematoma in the occipital region. Thepatient was conservatively treated.Computerised tomography taken four weeks after injury showed partial resolution of theinterhemsipheric hematoma and full resolution of the intracerebral hematoma. Controltomography taken one year later was normal. Interhemispheric subdural hematoma is a rareclinical entity. A close clinical observation and medical treatment is advocated as anappropriate treatment modality in stabilized or improving patients.
