Creutzfeldt-Jakob-like EEG Changes in a Case of Fatal Lithium Toxicity

Özet

Creutzfeldt-Jakob benzeri EEG bulguları olan bir fatal lityum toksisitesi olgusu sunulmuştur.52 yaşında kadın hasta, bilinç bozukluğu, ateş ve jeneralize nöbetler ile başvurdu. Hastanınbipolar afektif bozukluğu vardı ve yaklaşık 30 yıldır lityum ve klorpromazin tedavisialmaktaydı. Ailesinden, hastada bir aydır anoreksi, bulantı, kusma, diyare ve uykusuzluk gibilityum toksisitesinin sistemik belirtileri olduğu öğrenildi. Nörolojik muayenede uykuya meyil,jeneralize hipotoni ve hiporefleksi vardı. Laboratuvar incelemelerinde lökositoz, hiperglisemi,renal ve tiroid fonksiyonlarında bozukluk saptandı. Beyin omurilik sıvısı incelemeleri vekraniyal bilgisayarlı tomografi normaldi. EEG'de yaygın yavaş zemin aktivitesi ve bir kısmitrifazik özellikte periodik keskin dalga deşarjları görüldü. Kan lityum seviyesi 1.88 mEq/Lbulundu. Tekrarlanan hemodiyalize rağmen, hasta komaya girdi ve başvurusundan bir haftasonra kaybedildi. Bu olgu, Creutzfeldt-Jakob benzeri EEG bulguları olması ve lityum seviyesinormal seviyeye düşmesine rağmen fatal seyir göstermesi nedeniyle nadir bir lityumtoksisitesi örneğidir.

Abstract

We present a case of fatal lithium toxicity with EEG changes suggesting Creutzfeldt-Jakobdisease. A 52-year-old woman was admitted with declined consciousness, fever, andgeneralized seizures. She had bipolar affective disorder and was on lithium andchlorpromazine treatment for about three decades. Her family described that she had systemicsymptoms of lithium toxicity such as anorexia, nausea, vomiting, diarrhea, and insomnia forone month. Neurological examination disclosed declined consciousness, generalizedhypotonia and hyporeflexia. Laboratory examinations revealed leukocytosis, hyperglycemia,and renal and thyroid dysfunction. Cerebrospinal fluid examination and cranial computerizedtomography were unremarkable. The EEG demonstrated widespread slow wave activity andruns of periodic sharp waves, some with a triphasic pattern. Blood level of lithium was 1,88mEq/L. In spite of repeated hemodialyses, she became comatose and died one week afteradmission. This case represents an uncommon presentation of lithium toxicity withCreutzfeldt-Jakob like EEG changes and fatal outcome inspite of normalization of lithiumlevels.
