Spontaneous Intracranial Bullet Migration: A Complication of Penetrating Cranial Gunshot Wound: Case Report and Review of the Literature

Özet
Beyine ateşli silahla kraniyal delici yaralanmada kurşunun kendiliğinden migrasyonu bilinenender komplikasyonlardan biridir. Yazarlar 55 yaşında bir erkek hastada delici kranyal ateşlisilah yaralanmasında göçeden kurşun olgusunu sunmaktadırlar. Hastada ağır bir sağhemiparezi ve sensoriyelafazi mevcut idi. Kafatası röntgeninde tesbit edilen kurşunçekirdeğinin BT ile sol parieto-oksipital bölgede olduğu gözlendi. Hasta opera edilmedenkonservatif olarak tedavi gördü. Kontrol BT tetkikinde kurşun çekirdeğinin 17 gün sonra göçederek sol oksipital pole superior sagital sinüs ve Torcular Herofili komşuluğuna yerleştiğigözlendi. Hastanın sağ tarafındaki güçsüzlükte belirgin bir düzelme olmasına karşınkonuşmasında hem motor hem de sensoriyel bozukluğu sürdü. Delici kraniyal ateşli silahyaralanmasında kurşun çekirdeğinin spontan migrasyonunun olabileğini göstermesi açısındanbu olgu yönetininde kararların kişisel olarak alınmasını gündeme getirmektedir.
Abstract
Spontaneous migration of retained bullets in the brain is a recognized rare complication ofpenetrating cranial gunshot wounds to the brain. The authors report one rare case in a 55-yearold male patient who presented with a penetrating cranial gunshot wound and a retainedbullet. He had a dense right hemiplegia and expressive aphasia on admission. Skull x-rayconfirmed the retained intracranial bullet, which on CT scan was located at the left parietal- occipital region. He was managed conservatively. A repeat CT scan 17 days post injuryshowed spontaneous migration of the bullet to the left occipital pole adjacent to the superiorsagittal sinus and near to the Torcula Herophili. He had a remarkable improvement of hisright sided weakness but had severe receptive and expressive language deficits. This casehighlights the need to watch out for the occurrence of spontaneous migration of intracranialbullets after a penetrating cranial gunshot wound and when detected management decisionsshould be individualized.
