Proton Magnetic Resonance Spectroscopy and Intraoperative MRI Navigation for Frameless Biopsy

Özet
Proton manyetik rezonans spektroskopi (1H-MRS) patolojik doku ile ilgili metabolikdeğişiklikleri ya da biyokimyasal bilgileri sağlayan hızlı, invaziv olmayan bir yöntemdir.Dahası, heterojen yapıya sahip gliomlarda en iyi biyopsi sahasını göstermesi açısındankullanılabilir. Bu makalenin amacı kafaiçi lezyonlarının çerçevesiz biyopsisinde intraoperatifManyetik Rezonans Görüntüleme (iMRG) ve Proton Manyetik Rezonans Spektroskopi (1H- MRS)'nin birlikte uygulanmasının klinik etkinliğinin değerlendirilmesidir. Operasyonlarsırasında 1H-MRS, lezyonun metabolizmasını analiz ve en uygun biyopsi noktasını tesbitetmek için kullanıldı. Çerçevesiz biyopsiler için hedef nokta tesbiti 1h-MRS'nin gösterdiği enyüksek kolin/NAA oranı seçilerek gerçekleştirildi. Navigasyon sistemi ve 1,5T iMRG yardımıile ki bize beyin sapması olayına bir çözüm olacak tarzda patolojik tanı elde amacı ile otuz- bir beyin biyopsi işlemi gerçekleştirildi. Tüm hastalarda (n=31) çerç evesiz olarak alınanbiyopsilerden 30 hastada patolojik tanı kondu ve başarı oranı %96,8 (30/31) oldu. Biyopsisonrası hiçbir hastada yeni nörolojik defisit ya da intraserebral kanama gibi ciddikomplikasyon gelişmedi. Böylece yazarlar 1h-MRS'nin en uygun biyopsi noktasını saptamadave iMRG navigasyon yardımı ile biyopsinin hatasız alınabileceği kanısına vardılar.İntraserebral lezyonların çerçevesiz biyopsisi için 1H-MRS ve iMRG navigasyonkombinasyon uygulaması teknik olarak kolay ve güvenilir bir yöntemdir.
Abstract
Objective: To assess the clinical efficacy of the combined application of Proton MagneticResonance Spectroscopy (1H-MRS) and intraoperative Magnetic Resonance Imaging (iMRI)navigation in frameless biopsy of intracranial lesions.Method: 1H-MRS was used to analyze the metabolism of the lesions and to locate appropriatebiopsy point. We selected target points with the highest Cho/NAA ratio as shown by 1H-MRSfor frameless biopsies. Thirty-one cerebral biopsy procedures were performed to identify thepathological diagnosis with the help of a 1.5T iMRI and a navigation system. After thebiopsy, we routinely injected 0.5-1.0ml of air to the target zone through the biopsy needle. Itcan either serve as a marker and confirm that the targeted tissue has already been acquired, orform an air plug for local hemostasis.Results: Frameless biopsies were successfully performed in all 31 cases, and pathologicaldiagnosis were made in 30 patients, with a success rate of 96.8% (30/31). None of the patientssuffered from significant complications, such as intracranial bleeding or new neurologicaldeficit.Conclusion: 1H-MRS assists in finding the best biopsy lesion point and iMRI can guidebiopsy accurately. The combined application of 1H-MRS and iMRI navigation is technicallyfeasible and safe for frameless biopsy of intracerebral lesions.
