Single-fiber EMG in Meniere's Disease

Özet
Amaç: Kesin etyolojisi bilinmeyen ve bir kanallopati hipotezi önerilmiş olan Menierehastalarında motor son plak fonksiyon ( MSP) işlevini değerlendirmek.Yöntem: Meniere hastalığı tanısı almış 20 (13 kadın, 7 erkek) ve sağlıklı normal kontrol 12(4 kadın 8 erkek) olgu, Extensor Digitorum Communis kasına istemli kasılma sırasında teklif-EMG , üst ve alt ekstremite sinir iletim çalışmaları ve Extensor Digitorum Communiskasına konsantrik iğne EMG ile değerlendirildi. Her bir MD olgusu için ortalama jitterdeğerleri ölçüldü ve bu değerler normal kontrollerden elde edilen değerler ile karşılaştırıldı.Bulgular: Meniere hastalarının ortalama jitter değerleri normal kontrol olgularınındeğerlerinden farklı bulunmadı.Sonuç: Bulgularımız nöromuskuler ileti defekti ile uyumlu olmasa bile MD patogenezindekanal disfonksiyonu varlığını dışlamaz.
Abstract
Objective: To assess neuromuscular transmission (NMT) in patients with Meniere's disease(MD), in which the precise etiology is unknown and a channelopathy hypothesis has beenproposedMethods: Single fiber electromyography (SFEMG) during voluntary contraction of theextansor digitorum communis muscle, nerve conduction studies of upper and lowerextremities and concentric needle electromyography of the extensor digitorum communiswere performed on 20 patients (13 female, 7 male) with MD and 12 age-matched normalcontrols (4 female, 8 male). Mean jitter value was calculated for each subject and wascompared with the values recorded from normal controls.Results: The mean jitter values of the MD group were not different from the mean jittervalues of normal control subjects.Conclusion: Even though our findings are not consistent with neuromuscular conductiondefect, this doesn t exclude presence of channelopathy in the pathogenesis of MD.
