Primary Progressive Aphasia With Motor Neuron Disease: A Case Report

Özet
Frontotemporal demansın alt grubu olan primer progresif afazi, nadir görülen ve 2 yılboyunca sadece dil fonksiyonlarında ilerleyici bozulmaya sebep olan dejeneratif birhastalıktır. Amyotrofik lateral skleroz (ALS) ise klinik olarak üst ve alt motor nöronbulgularıyla karakterize, beyin ve omuriliğin üst ve alt motor nöronlarında belirgindejenerasyonun eşlik ettiği progresif nörodejeneratif bir hastalıktır. Frontotemporal demansile ALS birlikteliği literatürde bildirilen bir tablo olmasına rağmen, bu tabloya göre daha hızlıseyir gösteren ve motor afaziyle başlayıp motor nöron hastalığının klinik tabloya eklendiğiprimer progresif afazi ve ALS birlikteliği son yıllarda bildirilmeye başlanmış ve ayrı bir klinikantite olarak değerlendirilmesi gerektiği öne sürülmüştür. Her iki hastalığın da etkin birtedavisi henüz bulunmamaktadır. Kliniği primer progresif afazi ile başlayıp 2 yıl sonra motornöron hastalığının tabloya eklendiği olgumuzu Positron Emisyon Tomografi (PET) verileri ilesunduk.
Abstract
Primary progressive aphasia is a sub-group of frontotemporal dementia and a degenerative,rarely observed disease that only causes progressive deterioration in language functions fortwo years. Amyotrophic Lateral Sclerosis (ALS) is a progressive neuro-degenerative disease,clinically characterized with upper and lower motor neuron findings, accompanied bydistinctive degeneration in upper and lower motor neurons of the spinal cord. Frontotemporaldementia together with ALS is a case stated in the literature. However, a faster progressingprimary progressive aphasia, initiating with motor aphasia and with addition of motor neurondisease to the clinical situation, together with ALS was reported in recent years. Therefore, itwas proposed to be assessed as a separate clinical entity. Both these diseases do not have aneffective treatment yet. Our case is clinically initiating with primary progressive aphasia andtwo years later motor neuron disease started. The case is presented with a positron emissiontomography (PET) data.
