Acute Cerebellar Infarction After Percutaneous Vertebroplasty

Özet

Perkütan vertebroplasti (PV) ağrılı kompresyon fraktürlerinin tedavisinde kullanılan güvenilir,etkili ve çok ender olarak komplikasyonlara yol açan bir işlem olarak bilinmektedir. Bilgimizegöre PV sonrası komplikasyon olarak gelişen serebral infarkt olgu sunumu yalnızca buolgudur. Burada 82 yaşında bir kadın hastada işlem sonrası (6 saat sonra) ender olarakvertebral arter tıkanması sonucu gelişen akut sol serebellar infarkt olgusu sunmaktayız. Beyinmanyetik rezonans (MR) angiografi sol vertebral arterde tam tıkanma gösterdi. Hastaya tutucutedavi uygulandı. Bilgimize göre PV sonrası akut serebellar infarkt geçiren bir olgu şimdiyedeğin literatürde bildirilmemiştir. Embolizm olasılığını hastanın işlem sırasında yüz aşağıpozisyonda yattığını ve başının çevrili olduğunu düşünürsek dışarıda tutamayabiliriz ama PVve akut arterial infarkt arasında hiçbir özel bağ bulunmamaktadır. Böylece bukomplikasyonun nedenselden çok tamamen rastlantısal olduğu söylenebilir. Yine de yaşlıhastalarda postoperative değişken durumlara özel dikkat gösterilmesi gerekir.

Abstract

Percutaneous vertebroplasty (PV) is considered a very safe and effective procedure to treatpainful compression fractures, and this procedure rarely results in complications. To ourknowledge, there is only one case report of the complications associated with cerebralinfarction after PV. Here, we report a rare case of postoperative acute cerebellar infarctionthat was caused by vertebral artery occlusion. We present a rare case of left acute cerebellarinfarction that developed approximately 6 h after PV in an 82-year-old woman. Brainmagnetic resonance (MR) angiography showed complete occlusion of the left vertebral artery.The patient was treated conservatively. To the best of our knowledge, acute cerebellarinfarction after PV has yet not been reported. Although we cannot rule out the possibility ofembolism because the patient was in the prone position with her head rotated during theoperation, there are no specific links between acute arterial infarction and PV. Thus, thiscomplication may be incidental rather than causal. Nevertheless, special attention should bepaid to the postoperative variable changes in aged patients.
