Spontaneous Rhinorrhea Associated with Defect at Cribriform Plate and Orbital Roof Seen with Primary Empty Sella Syndrome: Case Report

Özet
Genellikle travmadan ve cerrahi girişimlerden sonra, nadir olarak da spontan olarak görülenrinore, tedavi edilmediği takdirde mortal komplikasyonlara yol açabilmektedir. Travmatikrinore genellikle cerrahi tedavi gerektirmeden durmakta ancak bu durum non-travmatik rinoreiçin geçerli olmadığından cerrahi girişim gerektirmektedir. Spontan rinore sıklıkla primerempty sella ile birlikte olmakta ve bu hastalarda çoğunlukla paranazal sinüs duvarlarında vekafa tabanında kemik defektler saptanmaktadır. Bu olgularda genellikle empty sella gibiintrakranyal basınç artışı bulguları olsa da spontan rinore, intrakranyal basınç artışı olmadanda görülebilmektedir. Biz burada; 46 yaşında, berrak burun akıntısı yakınmasıyla başvuran,empty sella ile birlikte kribriform plate ve orbita tavan defekti saptanıp spontan rinoretanısıyla cerrahi olarak tedavi edilen bir kadın olguyu sunduk. Hastanın postoperatif onsekizaylık takiplerinde rinore görülmedi.
Abstract
Rhinorrhea which is usually seen after trauma and surgical interventions but rarely seenspontaneously may lead to mortal complications if not treated. Traumatic rhinorrhea usuallyceases without surgery however since this is not true for non-traumatic rhinorrhea, it requiressurgical intervention. Spontaneous rhinorrhea is generally associated with empty sella and inthese cases osseous defects are detected frequently in the walls of the paranasal sinuses andskull base. Although in these cases signs of raised intracranial pressure such as empty sellaare usually seen, rhinorrhea can also be seen without raised intracranial pressure. Here wereport a 46-year old woman presented complaining of clear fluid leaking from her nose whohad defect of cribriform plate and orbital roof together with empty sella and was operatedwith the diagnosis of spontaneous rhinorrhea. In course of postoperative follow-up foreighteen months no rhinorrhea was seen.
