Özet
Amaç: Yüksek mortalite ve morbidite riski olan status epileptikus hızla müdahale edilmesigereken nörolojik acil tablolardan biridir. Standart tedavide kullanılan ilaçların birçoğusistemik yan etkilere sahiptir. Levetirasetam (LEV) karaciğer üzerinden metabolize olmayanve ilaç etkileşimi minimal olan yeni antiepileptik ajanlardan biridir. Özellikle intravenöz (İV)formunun dirençli nöbet ve status tablosundaki hastalarda kullanımı ile ilgili olumludeneyimler bildirilmektedir.Yöntem: Çalışmamızda nöroloji acil poliklinik ve konsültasyon birimi tarafından statusepileptikus ön tanısı ile İV LEV uygulanan 23-78 yaşları arasında 12 hasta retrospektif olarakdeğerlendirilmiştir. Hastalar klinik özelliklerine göre sınıflandırılmış ve İV diazepamtedavisine cevap vermeyen olgulara 1000-2000 mg. doz aralığında İV LEV uygulandığıgörülmüştür.Bulgular: Sistemik sorunları olan ve servislerde izlenen hastaların bu tedaviye çok iyi yanıtvermediği, buna karşın acile başvuran ve zaten antiepileptik kullanmakta olan hastaların isebelirgin bir şekilde faydalandığı izlenmiştir.Sonuç: İV LEV ile ilgili deneyimimiz sınırlı sayıda hasta ile olsa da literatürde daha öncebildirilen az sayıda olgu ve çalışmalar ile benzerlik göstermektedir. Status epileptikustedavisinde İV LEV ile yapılmış Türkiye'deki ilk çalışma olması nedeniyle deneyimlerimizipaylaşmak istedik.
Abstract
Objective: Status epilepticus is a neurological emergency feature with a risk of high mortalityand morbidity which requires immediate intervention. Many of the medications used instandard treatment have systemic side effects. Levetiracetam (LEV) is one of the newantiepileptic agents which are not metabolized through the liver and have minimal druginteraction. Positive outcomes with intravenous (IV) form of LEV on patients with refractoryseizures and status have been reported.Methods: In this study, we evaluated 12 patients with status epilepticus between the ages 23and 78 who were administrated IV LEV in the emergency department of Neurology,retrospectively. Patients were classified according to the clinical characteristics of theirseizures and IV LEV was given to patients who did not respond to intravenous diazepam inthe dose range 1000-2000 mg.Results: It has been observed that patients with systemic problems who were admitted to theNeurology service did not respond very well to IV LEV treatment whereas the patients on antiepileptic drugs who were referred to the emergency department benefited significantlyfrom LEV.Conclusions: Although our study involves a limited number of patients on whom effects IVLEV were examined, our results concur with previous reports similarly small number of casesin the literature. The first study of its kind in Turkey, we would like to contribute to literaturewith our experience of IV LEV in status epilepticus.
