Clinical Features and Treatment of Spinal Myxopapillary Ependymoma: A Series of 15 Cases

Özet
Giriş: Miksopapiller ependimom hemen her zaman filum terminaleden gelişen ender görülenbir ependimomdur. Spinal miksopapiller ependimom (SME) ise daha da enderdir. Bununlabirlikte,şu ana kadar SME için ideal bir tedavi şekli yoktur.Yöntem: Hastalar Ocak 2000den Aralık 2009a kadar cerrahi olarak eksize edildiler. Kliniközellikler, tanı, tedavi, sonuç ve izleme raporları 15 hasta için histopatolojik olarak SMEtanısı ile retrospektif olarak çalışıldı.Sonuçlar: Çalışmaya dahil edilen hastaların 11I erkek ve 4ü kadındır. Yaş ortalaması 36,5olmak üzere 13 ila 57 yaşlar arasındadır. İzlem süresi ameliyat tarihi ile hastanın en songörülen klinik muayenesi ile değerlendirildi. Histolojik inceleme derece I miksopapillerpapillom olarak gösterildi. Hastaların yedisinde tek parça halinde total eksizyon, 2 hastadaparçalar halinde total eksizyon ve 6 hastada ise subtotal eksizyon uygulandı. Sekiz hasta 1 yılsonra Frankel Derece E olarak nörolojik değerlendirmesi korundu. Rekürrens nedeniyleopere edilen üç hastada subtotal eksizyon sonrası düzelme olmadı. İzlem sırasında tümörrekürrensi olmadı. Bu olgu serisinde ölüm olmadı.Yargı: SMEnin başlangıcı tipik değildir, bu nedenle erken tanı önem taşır. Cerrahi SMEninrutin tedavi şeklidir. Tek parça halinde total eksizyon, parçalar halinde total eksizyon ya dasubtotal eksizyondan daha iyi nörolojik sonuç verir. Subtotal eksizyon yine bu hastalıktanrekürren gelişen hastalarda yapılmıştır. Son izlem kontrollerine değin bir düzelmegöstermediler. Bu çalışma SME araştırma ve tedavisi için daha ileri yönlenmeler öngörmektedir.
Abstract
Background: Myxopapillary ependymoma is a rare ependymoma arising almost exclusivelyfrom the filum terminale. Spinal myxopapillary ependymoma (SME) is even rarer. However,up to now there has been no ideal treatment for SME.Methods: The patients received surgical removal from January 2000 to December2009.Clinical presentation, diagnose, treatment, therapeutic outcome and follow-up results of15 patients, who were pathologically diagnosed with SME, were retrospectively studied.Results: The patients included eleven males and four females, with an age range of 13 to 57years (mean 36.5 years). The length of follow-up was determined from the date of surgery tothe patient's most recent clinic visit. Histological examinations showed WHO gradeⅠmyxopapillary ependymomas. Gross total removal was achieved in seven patients,piecemeal total removal in two patients, and subtotal removal in six patients. Eight patientswere neurologically preserved with Frankel Grade E one year later. Three patients who hadrecurrence of the tumor for this operation had no improvement after the subtotal resection.There was no tumor recurrence during the follow-up. There was no death in this group.Conclusions: The onset of SME is not characteristic, so pristine diagnosis is very important.Surgery is the routine treatment for SME; gross total removal has better outcomes thanpiecemeal total removal and subtotal removal. Subtotal removal is achieved in the patientswho have the recurrence before this operation. They have no improvement until the recentfollow-up. This study provides some directions for further research and treatment of SME.
