Özet
Amaç: Epilepsili kadınlar %90'ların üzerinde sağlıklı bebek dünyaya getirirler. Ancakkullanılan antiepileptik ilaçlar (AEİ), maternal nöbetler, genetik ve sosyoekonomik nedenlerfetüsün sağlığını olumsuz yönde etkileyebilir. Burada epilepsi polikliniğimizdedeğerlendirilen epilepsili gebeler tartışılmıştır.Gereç ve Yöntem: Çalışma Haziran 2002-Ocak 2012 tarihleri arasında polikliniğimizde takipedilen ve takibimizde iken çocuk sahibi olan toplam 83 epilepsili hastanın, 87 gebeliğinikapsamaktaydı. Tüm vakaların demografik verileri, medikal ve epilepsi özgeçmişleri, gebeliktakipleri, kullanılan ilaçlar ve malformasyonlar değerlendirildi.Bulgular: Çalışmaya alınan hastaların yaş ortalaması 26.99±4.73 ve epilepsi süresi 8.49±6.23yıldı. Elli iki (%62.7) hastada fokal başlangıçlı nöbet (sekonder jeneralize olan veya olmayan)vardı. Kalan otuz bir olgu ise jeneralize nöbete sahipti. Elli bir gebelik planlı idi. Otuz altıgebelikte AEİ veya ilaç dozu gebelik öncesi değiştirildi, iki olguda ise AEİ kesimi sonrasıgebeliğe izin verildi. Yetmiş altı bebek sağlıklı iken, altı gebelik ilk trimasterda spontanabortusla sonlandı, bir gebede ise ektopik gebelik mevcuttu. Dört bebekte ise majormalformasyon saptandı (iki gastroşizis, bir ventriküloseptal defekt, bir meningosel). Spontanabortusların beşi ve malformasyonlu bebeklerin üçü takipsiz gebeliklerdi.Sonuç: Epilepsili kadınlarda gebelik planlı olmalı ve tedavi gebelik öncesi düzenlenmelidir.
Abstract
Objectives: More than 90 percent of pregnant women who have epilepsy deliver healthybabies. However, antiepileptic drugs (AED) that were used, genetic and socio-economiccauses of maternal seizures can adversely affect the health of the fetus. Here, epilepticpregnant women, who were followed up in our epilepsy outpatient department, werediscussed.Methods: This study was included eighty-seven pregnancies from 83 epileptic women whowere following in our epilepsy department and had a child between June 2002-January 2012.All demographic data, medical and epileptic history, data about pregnancy, antiepileptic drugsmalformations for each patient were evaluated.Results: The mean age of patients was 26.99 ± 4.73 and epilepsy duration was 8.49±6.23years. There was focal onset seizure (with or without secondary generalisation) in fifty-twopatients (62.7%). The remaining thirty-one patients had generalized seizures. Fifty-onepregnancies were planned. Before pregnancy, AED or drug dosage was changed in 36women, pregnancy was allowed after the discontinuation of AED in two patients. Sixpregnancies were ended with spontaneous abortus in the first trimaster and ectopic pregnancywas present in one patient, while seventy-six infants were healthy. Four patients had a majormalformation (two gastrochisis, one ventriculoseptal defect, one meningocele). Five patientsin spontaneous abortus and three patients who had major malformation infant were unplannedpregnancies.Conclusion: Pregnancy in women with epilepsy must be planned and the regulation oftreatment should be done before pregnancy.
