Difficult Diagnosis of an Atypical Amyotrophic Lateral Sclerosis Case with Foot-drop

Özet
Atipik başlangıçlı Amyotrofik Lateral Skleroz (ALS) olguları, klinisyenleri çoğu zamanteşhiste zorlamakta ve tanıda gecikmelere yol açmaktadırlar. Bu olgu sunumunda altekstremitede ağrı ve motor güçsüzlüğü olan, farklı zamanlarda birden çok EMG incelemeleride dahil olmak üzere 8 ay süresince tetkik edilen, lomber disk hernisi nedeni ile ameliyatettiğimiz hastamızı sunmaktayız. Motor aksonal nöropatinin ayırıcı tanısı çoğu zaman oldukçazor olabilmektedir. Amyotrofik Lateral Skleroz tansında elektromiyografik incelemeler(EMG) önemli yer tutmaktadır. EMG, Amyotrofik Lateral Skleroz tanısını destekleyen veALS'yi taklit edebilen hastalıkların dışlanmasına yönelik önemli bir tetkiktir. Klinik şüphehalinde hastalarda kesin tanıya ulaşmak için tekrarlayan EMG incelemeleri yapılmalıdır.
Abstract
Amyotrophic lateral sclerosis (ALS) with atypical symptoms poses a diagnostic challenge toclinicians, frequently resulting in delayed diagnosis. The patient reported here presented withunexplained lower extremity pain and weakness and he was investigated for 8 months, withan extensive work-up, including several EMG studies, especially to rule out canal stenosisprocesses, before a diagnosis of ALS could be made, largely on the basis of upper and lowermotor neurons. Differential diagnosis with motor axonal neuropathies may be particularlydifficult. EMG is an essential tool for the early diagnosis of ALS, which increases potentialbenefit s of therapeutic interventions. In motor neuron diseases/amyotrophic lateral sclerosis(ALS), electrodiagnostic techniques are essential in supporting the diagnosis and excludingother conditions that clinically resemble ALS. In case of clinical suspicion in patients withrecurrent EMG thinning should be done to reach the definite diagnosis.
