Revision Surgery After Inappropriate Posterior Fossa Decompression For Craniocervical Junction Malformation

Özet
Giriş: Kranioservikal bileşke malformasyonu için birçok tedavi seçeneği tanımlanmıştır.Yazarlar uygunsuz olarak aşırı posterior fossa dekompresyonu uygulanmış kranioservikalbileşke malformasyonunda intraoperatif BT-navigasyon sistemini de içeren bir revizyoncerrahi tekniği tanımlamaktadırlar.Yöntem: Başka kurumlarda 2009 Nisan ayı ile 2012 Mart ayı arasında aşırı posterior fossadekompresyonu uygulanan Chiari Tip I malformasyonu, baziler invaginasyonu,siringomyelisi olan 12 hasta revizyon cerrahisine tabi tutuldu. Klinik, tanı, tedavi, sonuç veizlemleri retrospektif olarak çalışıldı.Sonuç: Sekiz hastaya redükte pozisyonda posterior fiksasyon ve füzyon, redükte olmayankraniovertebral bileşkesi olan üç hastaya sadece posterior oksipitoservikal fiksasyon, birhastaya da hem posterior oksipitoservikal fiksasyon hem de ikinci bir işlem olarak transoralodontoidektomi uygulandı. Radyolojik olarak bölgenin postoperatif stabilizasyonu vebeyinsapı basısının dekompresyonu gösterildi ve onbir hastada semptomların iyileştiğisaptandı. İzlem sürecinde ( 9-24 ay) sırasıyla tüm hastalarda oksipital kemik ile oksipitalkemik arasında solid kemik füzyonu izlendi.Yargı: Chiari Tip I malformasyonu ile birlikte olan baziler invaginasyon olgularında aşırıgeniş posterior fossa ve üst servikal kanal dekompresyonundan kaçınmak gerekir.İntraoperatif BT ve nöronavigasyon kullanarak oksipitoservikal fiksasyon ve füzyonuygulanması böyle uygunsuz aşırı posterior fossa dekompresyonlarını başarılı şekilde tedavieder. Posterior dekompresyon ve fiksasyon sonrası klinik semptomları düzelmeyen hastalardatransoral yolla ventral dekompresyon mutlaka düşünülmelidir.
Abstract
Background: A variety of treatment options have been described for craniocervical junctionmalformation. The authors described a revision surgical technique with intraoperative CTnavigation system for treatment of craniocervical junction malformation associated withinappropriate excessive posterior fossa decompression.Methods: The 12 patients with Chiari I malformation, basilar invagination, andsyringomyelia, which were performed extensive posterior fossa decompression in otherinstitutions received the revision surgery from April 2009 to March 2012.Clinicalpresentation, diagnose, treatment, therapeutic outcome and follow-up results of the patientswere retrospectively studied.Results: 8 patients were performed the posterior fixation and fusion in a reduced position. 3patients with irreducible craniovertebral junction were only performed the posterioroccipitocervical fixation, whereas 1 patient was performed the posterior occipitocervicalfixation, and a transoral odontoidectomy was performed as the second procedure. Thepostoperative stability of the region and decompression of the brainstem was confirmedradiologically, and the 11 patients were relieved of their symptoms. At 9-24 months offollow-up, respectively, solid bone fusion was observed between the occipital bone and axisin all patients.Conclusions: An excessive large posterior fossa and upper cervical canal decompressionshould be avoided for basilar invagination associated with Chiari I malformation. The revisionoccipitocervical fixation and fusion with the intraoperative CT and neuronavigation fortreatment of craniocervical junction malformation with inappropriate excessive posterior fossadecompression can be successfully performed. Ventral decompression via the transoral routeshould be considered in the patients whose clinical symptoms had not been improved after theposterior decompression and fixation.
