Surgical Treatment of Acoustic Neuroma Inside Internal Auditory Canals

Özet
Giriş: Bu makalede amaç tümör kitlesinin total eksizyonu ve yüz ile işitme sinirlerini azamikorumayı amaçlayan bir cerrahi yaklaşımı araştırmaktadırYöntem: Birbirinden ayrı toplam 43 hastadaki akustik nörinomlar retrospektif olarak gözdengeçirildi. Tüm olgularda suboksipital retrosigmoid yaklaşım uygulandı. Ameliyat öncesihastalara posterior kranial fossa ve petröz kemik ince kesit BT taraması yapıldı. Ameliyatsırasında 31 hastada internal oditör kanal (IAC) açıldı ve 8. kranial sinir elektrofizyolojikolarak monitörlendi. Sonuç: Otuzyedi hastada tümör total olarak çıkartıldı (&86,1). Geride kalan 6 olguda subtotalrezeksiyon yapıldı. Fasiyal sinir 41 olguda anatomik olarak korundu (%95,3). Dört olgufasiyal sinir işlevi olarak House-Brackmann (H-B) derece I, ve 24 olguda H-B dereceⅡ ilaⅢ, ve 4 olguda derece Ⅳ olarak saptandı. Dört hastanın ikisinde ameliyat öncesi işitmepostoperatif olarak da korundu. Postoperatif ölüm olmadı.Yargı: Aküstik nörinomların intraoperatif olarak rezeksiyonlarında her iki kranial sinir (Ⅶ veⅧ) patolojik anatomisine hakim olarak IAC içerisindeki seyirlerinin bilinmesi, mikrodrillemeişlemini ustaca kullanma, intraoperatif elektrofizyolojik monitörleme ve microcerrahiteknolojisi ile her iki kranial siniri koruma bu işlemin temel noktalarını oluşturur.
Abstract
Objective: This paper aims to investigate an approach to increase the total excision rate oftumor mass and the preservation rate of facial-acoustic nerve.Methods: A total of consecutive 43 patients with acoustic neuroma were reviewed in thisretrospective study. In all cases, the suboccipital retrosigmoid approach was used.Preoperatively, patients accepted thin layer CT scanning through posterior cranial fossa andos petrosum. Intraoperatively, the internal auditory canals (IAC) of 31 patients were opened;and electrophysiologic monitoring on the cranial nerve Ⅷ was progressed.Results: Thirty-seven cases of tumors were totally removed (86.1%), and the rest 6 cases hadsubtotal resections. Facial nerves were anatomically preserved in 41 cases (95.3%). Fourcases had a facial nerve function of House-Brackmann (H-B) grade I, and 24 cases were H-BgradeⅡ to Ⅲ, and 4 cases were Grade Ⅳ. Two in four patients having preoperative hearingretained hearing postoperatively. No postoperative death.Conclusion: To be familiar with the pathologic anatomy relationship between cranial nerveⅦ and Ⅷ inside IAC, to skillfully grasp the technique of microdrill, together with theapplication of intraoperative electrophysiological detection technology and microsurgicaltechnique, are the key points for the preservation of cranial nerve Ⅶ and Ⅷ when resectingacoustic neuroma tumors intraoperatively.
