Nerve Fascicles Localization and Fiber Types Quantification in Human Tibial Nerve and Its Muscular Branches

Özet

Giriş: Bacakta sinir yaralanmalarının tamirinde sıklıkla tibial sinir ve müsküler dalları vericiolarak kullanılmaktadır. Ancak bu sinir ve dallarının fasiküllerinin yerleşimi ve lif tiplerimiktar belirlemesi ayrıntılı olarak yeterince bilinmemektedir. Bu çalışma belirtilen soruyayanıt olmak üzere mikrocerrahiye yardımcı olacak gereksinimi karşılamak umudutaşımaktadır.Yöntem: İnsan kadavralarından ya da taze ampüte ekstremitelerden elde edilen yirmimüsküler dalları ile birlikte tibial sinir seti popliteal fossada hazır edildi. Örnekler donuk-kesitkullanılarak Karnovsky-Roots yöntem ile işlendi.Sonuç: Müsküler dalları ile birlikte tibial sinirin enine kesitlerinde somatosensoryel fasiküllerposteromedial bölümde, somatik motor fasiküller ise anterolateral bölgede bulundular. Tibialsinirin somatik motor lifleri diğer kas ldalları arasında en yoğun olaral soleus kasında tesbitedildi ve sayısı 5620,36±98,56 idi. Tibialis posterior dalında γ-ince myelinize lifler en fazlasayıda gözlendi.Yargı: Sinir yaralanmasında verici seçiminde ilk seçenek olarak soleus kas dalının tercihedilmesi gerekir. Ortak peroneal sinir ve dallarının onarımında tibialis posterior dalıkesilmemelidir. Bu çalışma sonucu ayrıca anastomoz için en ideal yerleşimin tibial sinirinanterolateral bölgesi olduğu da söylenebilir.

Abstract

Background: Tibial nerve and its muscular branches are often used as donors for repairingnerve injuries in the leg. However, the localization of nerve fascicles and a quantitativeanalysis of the type of nerve fibers in this nerve and its branches are not understood in detail.We studied this question hoping to provide guidance for microsurgery.Methods: Twenty sets of tibial nerve and its muscular branches in the popliteal fossa wereobtained from either human cadavers or freshly amputated limbs. Samples were frozen- sectioned and processed with Karnovsky-Roots' method.Results: On the cross sections of tibial nerve and its muscular branches, somatosensoryfascicles are mainly found in the posteromedial area and somatic motor fascicles in theanterolateral region. The number of somatic motor fibers in the soleus muscle branch is5620.36±98.56, the highest among other muscular branches of tibial nerve. The γ-thinmyelinated fiber in the tibialis posterior branch is the highest.Conclusions: The soleus muscle branch should be considered as the first choice whenselecting donors for nerve injury. The tibialis posterior muscular branch should not be cutduring the repair of the common peroneal nerve and its branches. Furthermore, our resultsalso suggested that the anterolateral region of tibial nerve is the optimal location foranastomoses.
