Özet

Başlıca optik sinirleri ve spinal kordu etkileyen nöromiyelitis optika (NMO), NMOIgG/antiaquaporin-4 antikor (Anti-AQP4 Ab) pozitifliği, kötü prognozu, oligoklonal bandnegatifliğiyle diğer demiyelinizan hastalıklardan ayrılır. Kesin NMO kriterlerini karşılamayanbirçok hasta NMO spektrum bozukluğu (NMOsd) olarak kabul edilmektedir. NMO'yudüşündüren kraniyal MRG lezyonları, tekrarlayan optik nörit, transvers miyelit, tekrarlayanbeyin sapı, hipotalamus veya ensefalopati belirtileri ve AQP4-IgG seronegatifliği olanhastalarda NMOsd tanısı için destekleyici bir kanıt olarak kabul edilebilir. Beyin sapı veservikal spinal kord tutulumu olan, NMO IgG pozitifliği saptanan ve NMOsd olarak kabuledilen bir olguyu literatür bilgileri eşliğinde sunduk.

Abstract

Neuromyelitis optica (NMO), or Devic's disease, which mainly affects the optic nerves andthe spinal cord, is differentiated from the other demyelinating diseases by presence of NMOIgG/antiaquoporin-4 antibody (Anti-AQP4 Ab), poor prognosis, and absence of CSFoligoclonal IgG bands. Patients who do not meet the criteria for definite NMO, are consideredas NMO spectrum disorders (NMOsd). Brain MRI lesions suggestive of NMO may beconsidered as an alternate supportive evidence for the diagnosis of NMOsd in patients withrecurrent optic neuritis, longitudinally extensive transverse myelitis (LETM), recurrentbrainstem, hypothalamic or encephalopathy symptoms, and AQP4-IgG seronegative status.We present a case with involvement of brainstem and cervical spinal cord and with NMO IgGpositivity who considered as NMOsd together with the relevant literature.
