The Impact of Intracranial Artery Disease and Prior Cerebral Infarction on Central Nervous System Complications After off-pump Coronary Artery Bypass Grafting

Özet

Giriş: Pompa-dışı Koroner Arter Bypas Greftleme (PDKABG) tekniğindeki gelişmeler,başarılı bir operasyon olma yanı sıra ilgi, eşlik edebilen inme ve bilişsel bozukluk gibinörolojik hasarlanma üzerine yoğunlaştı. Bu çalışmanın amacı postoperative santral sinirsistemi (SSS) komplikasyonlarının manyetik rezonans görüntüleme (MRG) ile gösterilebilenönceki geçirilmiş serebral infarkt ya da manyetik rezonans angiografi (MRA) ilegösterilebilen intrakraniyal arter hastalığı ile ilişkisini ortaya koymaktır.Yöntem: Çalışma modeli PDKABG uygulanan ardışık 55 hastadan (ortalama yaşı 64,59±8,86olan 40 erkek ) oluştu. Her hasta operasyondan 24 saat once nörolojik ve nöropsikolojik testlerden geçti. MRG eski ya da yeni iskemik lezyonları ve MRA da intrakraniyal arterhastalığının varlığı ve derecesini göstermek üzere operasyondan önce kullanıldı. Hastalaroperasyondan sonra 8 gün boyunca inme ve bilişsel bozukluklar yönü ile izlendi. Öncekiserebral infarkt ve intrakraniayl arter hastalığı gibi önceden bilinen potansiyel belirleyiciler ilepostoperative inme arasındaki ilişki tek-değişkenli yöntemlerle analiz edildi.Sonuç: Ellibeş hastanın ikisinde (%3,64) postoperatif inme gelişti ve hiçbir hastada bilişselbozukluk kaydedilmedi. Tek-değişkenli analizde postoperatif inme ile MRGde tesbit edilenönceki serebral infarkt (p=0,378) ve MRAda tesbit edilen intrakraniyal arter hastalığı(p=0,103) arasında bir ilişki saptanamadı.Yargı: Bu sonuçlar PDKABG sonrası inmede, intrakraniyal arter hastalığı ve önceki serebraiinfarktın bağımsız risk faktörleri olmadığını göstermektedir. Bununla birlikte küçük bir hastagrubu örneklemesi olan bu çalışma gibi bu ilişkileri araştıran daha ileri çalışmalar gereklidir.

Abstract

Background: Improvements in off-pump coronary artery bypass grafting (OPCABG) havefocused interest on neurological injuries, such as stroke and cognitive decline, which mayaccompany an otherwise successful operation. The purpose of this study was to determinewhether postoperative central nervous system (CNS) complications was related to priorcerebral infarction demonstrated by magnetic resonance imaging (MRI) or intracranial arterydisease demonstrated by magnetic resonance angiography (MRA).Methods: The study sample comprised 55 consecutive patients (40 men; mean age,64.59±8.86 years) who underwent OPCABG. Each patient underwent neurological andneuropsychological examinations 24 hours before surgery. MRI was used to identify oldand/or new ischemic lesions before surgery and MRA was used to determine the presence andseverity of intracranial artery disease. Patients were followed 8 days after surgery andevaluated for the development of stroke or cognitive dysfunction. Associations betweenpostoperative stroke and potential predictors, including prior cerebral infarction andintracranial artery disease, were analyzed using univariate methods.Results: Two of 55 (3.64%) patients had postoperative stroke and no patient showedcognitive decline. Univariate analysis found no association between postoperative stroke andprior cerebral infarction detected by MRI (P=0.378) or intracranial artery disease detected byMRA (P=0.103).Conclusion: Our results suggest that intracranial artery disease and prior cerebral infarctiondetected on MRI are not independent risk factors for stroke after OPCABG. However, giventhe small sample of patients included in this study, further investigation of these associationsis necessary.
