Nosocomial Infections Caused by Jugular Central Venous Catheterization in Brain Surgery Intensive Care Unit

Özet

Amaç: Beyin Cerrahi Yoğun Bakım Ünitesinde (BCYBÜ), santral venözkatetere (SVK) bağlıgelişen nazokomiyalenfeksiyonları incelemek ve alınabilecek önlemleri tartışmakamaçlanmıştır.Gereç ve Yöntem: Bu çalışma, GATA Haydarpaşa Eğitim Hastanesi Beyin ve Sinir CerrahiServisi YBÜ' de 2008 2011 yılları arasında yatan toplam 426 hasta üzerinde yapıldı.Çalışmada, SVK takılı olan toplam 145 hastanın verileri geriye dönük olarak hasta dosyalarıve Hastane Enfeksiyon Kontrol Komitesi kayıtları incelenerek kaydedildi.Bulgular: BCYBÜ' nde tedavi gören toplam 426 hasta takip edilmiş ve 145 hastaya SVKuygulaması yapılmıştır. Bu hastalardan 98' i asıl tanısına yönelik olarak cerrahi tedaviyealınmış ve SVK'i ameliyathane ortamında takılmıştır. Geri kalan 47' hasta BCYBÜ' de takipedilerek SVK' i YBÜ şartlarında takılmıştır. Hastalarda görülen SVK kaynaklı enfeksiyonajanlarından Metisiline dirençli koagülaz-negatif stafilokoklar) %25 (n=10) ve MetisileneDirençli Staphylococcusaureus %16 (n=4) ilk sıralarda yer almaktadır.Sonuç: Bu çalışmada cerrahi uygulanan ve ameliyathane ortamında kateter takılan olgularınSVK ile ilişkili kan dolaşım enfeksiyon oranları daha yüksek saptanmıştır. Cerrahi stresinimmünsüpresyona ve dolayısı ile enfeksiyonlara yatkınlığı arttırabileceği değerlendirilmiştir.

Abstract

Objective: The purpose of this retrospective study was to investigate nosocomial infectionsassociated with central venous catheter (CVC) in Neurosurgery Intensive Care Unit (NSICU)and to discuss the means of prevention.Material and Methods: The patients who were admitted to Neurosurgery ICU at the GATAHaydarpasa Teaching Hospital between the years of 2008 to 2011 were evaluated for thestudy. All the patient medical records that were collected at the Hospital Infection ControlCommittee were screened retrospectively. Of the 426 patients' medical records that werescreened, 145 patients with CVC were included in this study.Results: A total of 145 patients who were treated with CVC in NSICU were followed up. Ofthe 145 patients, 98 were surgically treated for main diagnosis and their CVC was insertedunder operation room conditions. The remaining 47 patients were followed up in NSICU andtheir CVC was inserted in the environment of ICU. Methicillin-resistant coagulase-negativestaphylococci (25%, n= 10) was the most commonly isolated agent from the patients withCVC-induced infections, followed by Methicillin-resistant Staphylococcus aureus (16%, n=4).Conclusion: In this study, the rate of CVC related infection of blood circulation was higher inthe patients who underwent surgery and whose catheter was inserted in the operation room. Itcan be postulated that surgical stress might result immunosuppression that predisposes CVCpatients to infections.
