Migration of a Lumbar Rod to Posterior Sacral Region: An Extremely Unusual Complication of Spinal Instrumentation

Özet

Posterior transpediküler enstrümantasyon spondilolistezis tedavisinde sıklıkla kullanılan biryöntemdir. Lomber rodun posterior sakral bölgeye migrasyonu son derece nadir vealışılmadık bir tablodur. Bu yazıda lomber rodun posterior sakral bölgeye migrasyonu sözkonusu olan 73 yaşında bir kadın hasta sunulmaktadır. Hastaya dört sene önce L4-L5spondilolistezis tanısı ile posterior transpediküler vida-rod enstrüman sistemi yerleştirilmiştir.Hastaya çekilen anteroposterior ve lateral direkt grafilerde sol taraftaki rodun fiksasyonnoktalarından tamamen kurtularak aşağı doğru sakral bölgeye migre olduğu görülmüştür. Buolgu nedeniyle, posterior fiksasyon olgularında dikkatli cerrahi teknik ve izlem konusundakiönemli noktalar vurgulanmıştır.

Abstract

Posterior transpedicular instrumentation is commonly used for the treatment ofspondylolisthesis. Migration of the lumbar rod to posterior sacral region is extremely unusual.In this report, a 73-year-old-female patient with the migration of lumbar rod to posteriorsacral region is presented. She had been placed a posterior trans-pedicular screw-rodinstrumentation system for L4-L5 spondylolisthesis four years ago. The anteroposterior andlateral lumbosacral roentgenograms revealed that left-sided rod released completely from thefixation points and migrated down caudally to the sacral area. This case report emphasizes theimportance of careful surgical technique and long-term follow up for the cases that haveundergone spinal instrumentation surgery.
