Evaluation of Risk Factors For Fever Onset as an Early Complication in Patients Who Suffered Acute Ischemic Stroke

Özet
Amaç: Bu çalışmanın amacı akut iskemik inmeli hastalarda erken komplikasyon dağılımındave seçici risk faktörlerinin etkisi olarak ateş başlangıcının değerlendirilmesidir.Yöntem: Akut iskemik inmeli 106 hasta çalışmaya dahil edildi. Hastalar ateşi olanlar ve ateşiolmayanlar olarak iki gruba ayrıldı. Yaş, cinsiyet, hipertansiyonun varlığı, anamnazde öncekiinme öyküsü, başvuru sırasında sistolik ve diyastolik kan basıncı, glisemi ve hemotokritseviyeleri, sedimentasyon, kolesterol ve lipid seviyeleri, Bamford sınıflamasına göre farklıinme tipleri, NIH inme skalasına (NINDS) göre işlevsel disabilite dereceleri gibi diğerparametreler değerlendirildi.Sonuç: Önceki inme olayı olan hastaların dağılımında (p=0.043) ve Bamford kliniksınıflandırmasına göre farklı inme tiplerinde (p=0.000) değerlendirilen gruplar ile ateş varlığıarasında istatiksel olarak anlamlı bir farklılık gözlendi. Ateşi olan hasta grubunda glisemiseviyelerinde (p=0.010), NIH inme skalası NINDS skorunda artış (p=0.000) ve önemli orandahemotokrit seviyelerinde azalma (p=0.028) saptandı. Akut inmeli hastalarda ateşbaşlangıcının önemli risk faktörleri şu şekilde sıralandı: önceki inmeler (p=0.048); artmışglisemi (p=0.041); azalmış hematokrit (p=0.035); Bamford skalasına göre inmenin kliniksınıflandırması (p=0.004) ve NIH inme skalası-NINDS işlevsel lezyon derecesi (p=0.000).Yargı: Post akut iskemik inmede erken komplikasyon olarak ateş başlangıcının riskfaktörleri: önceki inmeler, artmış glisemi seviyesi, düşük hematokrit değeri, etkilenen beyindokusunun iskemik zon boyutu ve NIH-inme skalası NINDSe göre işlevsel lezyonunyüksek derecesidir.
Abstract
Objectives: The aim of our study was to evaluate distribution and influence of selective riskfactors for a fever onset as an early complication in patients who suffered the acute ischemicstroke.Methods: The study included 106 patients who suffered acute ischemic stroke. Patients weredivided into two groups: group with fever and group without fever. Further parameters wereevaluated: age, gender, presence of hypertension, presence of previous stroke, systolic anddiastolic blood pressures at admission, glycemia levels, hematocrit levels, erythrocytesedimentation rate, cholesterol and triglycerides levels, distribution of different stroke typesaccording to Bamford classification, and degree of functional disability according to NIHstroke scale NINDS.Results: There is statistically significant difference in distribution of patients with previousstroke event (p=0.043) and different stroke types according to Bamford clinical classification(p=0.000) between evaluated groups regarding fever presence. Patients with fever hadsignificantly increased levels of glycemia (p=0.010), score on NIH stroke scale-NINDS(p=0.000) and significantly decreased hematocrit (p=0.028). Significant risk factors for thefever onset in patients who suffered acute stroke are: previous stroke (p=0.048); increasedglycemia levels (p=0.041); decreased hematocrit levels (p=0.035); clinical classification ofstroke according to Bamford scale (p=0.004) and degree of functional lesion on NIH strokescale NINDS (p=0.000).Conclusion: Risk factors for a fever onset as an early complication in post acute ischemicstroke are: previous strokes, increased glycemic levels, decreased hematocrit levels, the sizeof ischemic zone of affected brain tissue and higher degree of functional lesion according toNIH- stroke scale-NINDS.
