Chondromyxoid Fibroma of The Lumbar Spine. A Case Report and Literature Review

Özet
Giriş ve amaç: Kondromiksoid fibrom (KMF) sık görülmeyen bir iyi huylu tümör olarakkemik ve omur tutulumu enderdir. Bugüne değin literatürde lomber bölgede yerleşen sadeceon olgu bildirilmiştir. Ayırıcı tanısının ancak histopatolojik inceleme ile yapıldığı sıklıklatanısal karışıklıkların yaşandığı bir durumdur. Burada bel ağrısı ve sağ siyatalji sendromu ilegelen ve tam eksizyon ile cerrahi tedavisi yapılan bir lomber kondromiksoid fibrom olgusukısaca literatür eşliğinde sunulmaktadır.Yöntem: Bir yıldan beri süregelen bel ve siyatik ağrısı olan 65 yaşında kadın bir hastadır. MRgörüntülemede L4-5 seviyesinde epidural bir lezyon gözlendi. Preoperatif tanısı lomberartiküler oluşumun bir kisti idi. Kitle total olarak eksize edildi ve ardından füzyon yapıldı.Sonuç: Hastanın postoperatif dönemi olaysız ve ağrısı cerrahi sonrası hemen geçti. Duysal vemotor işlevler hemen hemen tamamen iyileşti. Cerrahiden 1 yıl sonra hastada herhangi birsemptom, bulgu ve nüks saptanmadı. Histolojik tanı KMF olarak bildirildi.Yargı: KMFun spinal tutulumu enderdir. Literatür taramamıza göre bu olgu lomber omurdagörülen onbirinci olgudur. Tanı temelde histopatolojik olarak konur. Nüks ve maligntransformasyon potansiyeli düşünüldüğünde mutlaka total en bloc olarak rezeksiyonuyapılmalıdır.
Abstract
Objective: CMF is an uncommon benign tumor of bone and spinal involvement is rare. Therewere only ten cases of CMF occurred in the lumbar spine reported in the literature to date. Itoften leads to diagnostic confusions and the differential diagnosis is based mainly on thehistopathologic examination. To present a rare case of chondromyxoid fibroma (CMF)involving the lumbar spine which presented with of low back pain and right-side sciatic pain,successfully treated by complete excision, with a brief overview of the literature.Methods: The patient was a 65-years old woman with persistent low back pain and sciaticpain of one year's duration. MR imaging demonstrated an epidural lesion at the L4/5 level.The preoperative diagnosis was cyst of lumbar articular process. Complete excision of thetumor and spinal fusion were performed.Results: The patient experienced immediate pain relief after surgery and the postoperativecourse was uneventful with near complete recovery of sensory and motor function. On 1-yearfollow-up after surgery, the patient reported no symptom such as pain or hyperesthesia and noevidence of recurrence were observed. Histologic analysis confirmed the diagnosis of CMF.Conclusion: The spinal involvement of CMF is rare. On the basis of a review of theliterature, this is the eleventh case reported which was occurred in the lumbar spine. Thediagnosis is based mainly on the histopathologic examination. Complete enbloc resectionshould be recommended in consideration of the potential of the recurrence and malignanttransformation.
