Neuroendocrine and Hemodynamic Effects of General Anesthesia and Spinal Anesthesia For Minimally Invasive Lumbar Disc Surgery: A Randomized Trial

Özet

Giriş: Spinal anestezi, elektif spinal cerrahi operasyonlarında genel anesteziye alternativeolarak kullanılabilir. Bu çalışmada minimal invaziv lomber disk cerrahisinde spinal anestezive genel anestezinin nöroendokrin ve hemodinamik etkilerini karşılaştırdık.Metod: Bu çalışmaya elli altı ASA I-II hasta dahil edildi. Cerrahi sonrası 30.dakika, 6.ve 24.saatlerde adrenokortikotropik hormon, kortizol ve glukoz için kan alındı.Hemodinamikparametreler kaydedildi. Daha sonra ilk idrar yapma, ilk mobilizasyon ve ilk analjezik ihtiyacızamanları, komplikasyonlar ve hasta memnuniyeti incelendi.Sonuçlar: Cerrahi sonrası 30. dakikada kortizol seviyesinde anlamlı farklılık saptandı(p=0.00). Monitörize edilen diğer kan değerlerinde farklılık saptanmadı (p>0.05). İlk idraryapma ve taburculuk zamanı spinal anestezi grubunda daha kısaydı.Tartışma: Sonuç olarak, bu sonuçlar tek mesafe lomber disk cerrahisi için spinal anestezigenel anesteziye iyi bir alternatif olabilir.

Abstract

Background: Spinal anesthesia used as an alternative to general anesthesia in lumbar spinalsurgical interventions could be implemented in all elective spinal surgical operations. Wecompared the neuroendocrine and hemodynamic effects of general and spinal anesthesia forminimally invasive lumbar disc surgery.Methods: Fifty-six patients classified as American Society of Anesthesiologists (ASA) I IIwere included in the study. Blood was drawn at 30 min and 6 and 24 h after surgery fordetermination of adrenocorticotropic hormone, cortisol, and glucose levels. Hemodynamicparameters were also recorded. Furthermore, the time of first voiding, the time of firstmobilization, the time of first analgesic requirement, complications, and patient satisfactionwere evaluated.Results: Significant differences were detected in cortisol values at 30 min after surgery(p=0.00). No significant differences were detected in the other blood values monitored(p>0.05). The time of first voiding and discharge time were shorter in the spinal group.Conclusion: As a result, our results show that spinal anesthesia can be a good alternative togeneral anesthesia for single-level lumbar disc surgery.
