Efficiency of Repeated Botulinum Toxin Type-A Injections in Post-Stroke Distal Limb Spasticity

Özet

Amaç: Bu çalışmada, distal üst ve alt ekstremite spastisitesi olan inmeli hastalardatekrarlanan A tipi Botulinum toxin (BT-A) enjeksiyonlarının birikici etkisini değerlendirmekamaçlanmıştır.Metodlar: Hastalar üst ve alt ekstremite spastisitesine göre iki grupta sınıflandırıldı.Spastisitenin derecesi modifiye Ashworth skalası(MAS) ile ölçüldü. Tedavi yararının globaldeğerlendirilmesi son vizitte hasta tarafından bildirilen belirtilerdeki düzelmeye göre çok iyi(%75-100 düzelme), iyi (%50-74 düzelme), orta (%25-49 düzelme) ve yetersiz (%25'inaltındaki belirti azalması) olarak oluşturuldu. Her vizitteki MAS skorları karşılaştırıldı. İnmesonrası 1 yıl içinde enjeksiyona başlanan hastalar erken tedavi grubu olarak kabul edildi.Erken ve geç tedavi grupları, BT-A enjeksiyonlarının etkinliği açısından karşılaştırıldı.Sonuçlar: Beşinci enjeksiyonun MAS skorları birinci enjeksiyonunkilerle karşılaştırıldığında,sadece ön kol pronatör kaslarında belirgin farklılıklar bulundu (p=0.021). MAS skorlarındakiazalma erken tedavi grubunda daha belirgindi. Tedavi yararının global değerlendirilmesi heriki grupta da yüksekti (üst ekstremite spastisite grubu: %27 çok iyi, %67 iyi; alt ekstremitespastisite grubu: %25 çok iyi, %58 iyi).Yorumlar: İnme sonrası distal üst ve alt ekstremite spastisitesi olan hastalarda düzenli olaraktekrarlanan BT-A enjeksiyonlarının yararında artış gözlendi. Fakat tedavinin birikici etkisisadece ön kol pronatörlerinde gösterildi.

Abstract

Objective: The purpose of this study was to assess cumulative effect of repeated Botulinumtoxin type-A (BT-A) injections in stroke patients with distal upper and lower limb spasticity.Methods: The patients were classified in 2 groups according to their upper and lower limbspasticity. The degree of spasticity was measured with the modified Ashworth scale (MAS).The global assessment of treatment benefit was done by patient-reported improvement insymptoms as very good (75-100% improvement), good (50-74% improvement), moderate(25-49% improvement ), and insufficient (less than 25% symptom relief) at the final visit.MAS scores of each visits were compared. The patients started on injections within 1 yearafter stroke were considered as early treatment group. Early and late treatment groups werecompared in terms of effectiveness of the BT-A injections.Results: When MAS scores of the 5th injections were compared to those of 1st injections, thesignificant differences were only found in forearm pronator muscles (p=0.021). MAS scoresdecrease was more pronounced in early treatment group. The global assessment of treatmentbenefit was high in both groups (the upper limb spasticity group: 27% very good, 67% good;the lower spasticity group: 25% very good, 58% good).Conclusions: An increase in the benefit of regular BT-A injections in post-stroke patientswith distal upper and lower limb spasticity was observed. But, the cumulative effect oftreatment was shown only in forearm pronators.
