Decompression of The Foramen Magnum Without Burr Hole For Treatment of Chiari Malformation Type 1: Report of 25 Cases

Özet

Amaç: Tip 1 Chiari Malformasyonu için bir çok cerrahi teknik bildirilmişse de, en iyi cerrahiyaklaşım halen tartışmalıdır. Bu klinik çalışmanın amacı Tip 1 Chiari Malformasyonu içinbasitleştirilmiş bir cerrahi tekniği ortaya koymaktır.Yöntem: 2004 ile 2009 yılları arasında ameliyat edilmiş yirmibeş Tip 1 ChiariMalformasyonu tanılı hasta çalışmaya dahil edildi. Tüm hastalarda önce C1 posterior arkıçıkarılmış, takiben Kerrison Ronguer yardımı ile foramen magnum kenarının alınması ilesuboksipital kraniektomi posterior fossada komplikasyondan kaçınılarak burr hole açılmadantamamlanmıştır.Sonuç: Çalışma grubunda ortalama yaş 36,8 ( 2769 yaş) olup, ortalama takip süreleri 16-36 ay ( ortalama 25 ay ) arasındaydı. Başağrısı en sık saptanan şikayetti. Tüm hastaların klinikkötüleşmeleri toparladı; 18 hasta ( % 72) tamamen iyileşirken, 6 hastada ( % 24) kısmiiyileşme gözlendi. Siringomiyeli beş (% 20) hastada radyolojik olarak devam ederken, 15 (%60) hastada sirinks kavitesi boyut olarak azalmış ve kalan beş hastada (% 20) tamamenkaybolmuştur.Tartışma: Posterior fossaya "burr hole" açılmadan uygulanan foramen magnumdekompresyonu güvenli ve kolay bir cerrahi tekniktir. Bu cerrahi teknik, Tip 1 Chiarimalformasyonu olan uygun hasta gruplarında önceki cerrahi tekniklerin yerinekullanılabilinir.

Abstract

Objective: Although several surgical techniques for Chiari Malformation Type 1 (CM1) havebeen described, the best surgical treatment is still controversial. The aim of this study was todescribe a simplified surgical technique for the treatment of patients with CM1.Methods: A total of 25 CM1 patients, admitted to our tertiary center constituted the studygroup. All cases underwent suboccipital decompression by removal of posterior lip offoramen magnum using Kerrison rongeurs after the resection of C1 posterior arch. Thecraniectomy was completed superiorly without making any burr holes for avoidingcomplication in all patients. Neurological examination was performed preoperatively and 1day, and 2, 6, and 12 months after surgery. Craniocervical and cervical spinal cord MRIs wereexamined and recorded for the presence of tonsillar descents and syringomyeliapreoperatively. A follow up MRI was performed two months after surgery.Results: The mean follow-up period was 65.02±16.94 (range, 46 to 94) months. Mostcommon symptoms and findings were occipital and neck pain radiating to the upper extremity(n=23, 92%), followed by numbness (n=22, 88%), sensory disturbance of the upper extremity(n=15, 60%), and vertigo (n=13, 52%). No major complications were observed in thepostoperative period. Clinical progression was halted in all patients; 18 (72%) patientsrecovered completely and six (24%) patients reported partial recovery. Syringomyelic cavitypersisted radiologically in five (20%) patients, decreased in size in 15 (60%) patients, andregressed completely in the remaining five (20%) patients.Conclusion: Foramen magnum decompression without burr hole is a safe and easy to performtechnique. This technique can be used almost in all cases instead of the commonly usedsuboccipital craniectomy technique with burr hole.
