A Clinical Analysis of Repairing the Whole Brachial Plexus Nerve Root Avulsion by Transferring C7 Nerve Root from the Uninjured Side

Özet
Bu çalışmanın amacı brakial pleksus sinir kökü tam avülsyonu olan hastalarda hasarsıztaraftaki C7 sinir kökünün transferini yaparak; etkilenen taraftaki iyileşmeyi gözlemekti. Adımlar halinde hasarsız taraftaki C7 sinir kökü transferi ile onarım yapılan brakial pleksussinir kökü tam avülzyonu olan 36 hasta seçildi. İzleme vizitleri yaparak, hasarlanmamıştaraftaki işlevleri kayıtlandırma, U.K. ile uyumlu kapsamlı değerlendirme yapıldı. Duysaldeğerlendirme için MRC skalası ve kas gücü için U.K. değiştirilmiş MRC skalası kullanıldı.Sonuçlar 2 hastada etkili supraspinatus kası iyileşmesi, 2 hastada deltoid kas gücü, 6 hastadabiseps kas gücü, 7 hastada triseps kas gücü, 4 hastada ekstensör karpi kas gücü ve 5 hastadafleksör karpi ile fleksör digitorum kas gücü iyileşmelerini gösterdi. Ameliyattan altı ay sonra7 hastada latissimus dorsi ve triseps kas gücü M5-ye, 10 hastada ekstensör digitorum kas gücüM5- ye yükseldi. Operasyondan 3 gün sonra 3 hastada hasarlanmamış tarafta Duysal DereceGS4e, 12 hastada GS3+ya, 12 hastada G3+ya, 11 hastada GS3e ve 10 hastada GS2yeyükseldi. Hastaların büyük kısmında operasyondan 1 yıl sonra iki-nokta ayırımı normale vesensoryel iyileşmeleri S4 seviyesine yükseldi.Brakial pleksus sinir kök avülzyonunda etkilenen tarafın işlevlerinin onarımındahasarlanmamış normal taraftaki C7 transferinin etkili ve uygun bir yöntem olarak yapılabilirve geçerli olduğu yargısına varıldı.
Abstract
The aim of this study is to perform a C7 transfer from the uninjured side for the patients withwhole brachial plexus nerve root avulsion, and observe the recovery outcomes of affectedlimb. The 36 patients with whole brachial plexus nerve avulsion who received a C7 transferfrom uninjured side for repair by stages were selected. Carrying out follow-up visits,evaluating limb functions on uninjured side, and making comprehensive assessment were inaccordance with U.K. MRC scale for sensation and U.K. amended MRC scale for muscularstrength. The results indicated that there were 2 patients with effective recovery ofsupraspinatus muscular strength, 2 of deltoid muscular strength, 6 of biceps brachii muscularstrength, 7 of triceps brachii muscular strength, 4 of extensor carpi muscular strength, and 5 offlexor carpi and flexor digitorum muscular strength. Six months after operation, there were 7patients whose latissimus dorsi muscular strength reached M5-, 7 triceps brachii muscularstrength reached M5-, and 10 extensor digitorum muscular strength reached M5-. Three daysafter operation, there were 3 patients whose limb on uninjured side reached Sensation GradeS4, 12 reached Grade S3+, 11 reached Grade S3, and 10 reached Grade S2. Majority ofpatients recovered to sensory function S4 and their two-point discrimination approximatelyrecovered to normal 1 year after operation. In conclusion, repair with C7 transfer from theuninjured side is currently an effective and feasible method to repair the functions of theaffected limb in brachial plexus nerve root avulsion.
