An Uncommon Headache at Our Doorsteps: Cerebral Venous Sinus Thrombosis From Combined Haemoglobinopathy and Protein S Deficiency, Presenting With Papilledema

Özet

Başağrısı yaygın ve çok yönlü bir rahatsızlıktır. Başağrısı nedenleri oransal olarak migren gibiselim bir durumdan ötürü olabildiği gibi hayatı tehdit eden bir anevrizmaya bağlı kanamayakadar uzanır. Tanısal zorluğu başağrısıile birlikte birçok semptomun birlikte görülmesinebağlıdır. Serebral venöz tromboz (CVST) genç erişkinlerde ve doğurganlık çağındakikadınlarda görülen ender bir serebral vasküler hastalıktır. Başağrısı, bazen KİBAS, iskemi,infarkt ya da subaraknoid aralığa kanamaya bağlı fokal nörolojik bulgularla prezante olabilir.Başağrısı en sık görülen semptomdur ve çoğu kez tek başınadır, bu nedenle de tanıyızorlaştırır. Tedavisi için kabul edilmiş bir protokol bulunmadığından CVST'nin tanısı gibiyönetimi de birçok risk faktörü nedeniyle zordur. CVST yüksek morbidite ve mortaliteyesahiptir. Protein S ve hemoglobin AC yetmezliği olan, 2 haftalık başağrısı ve ateşyakınmasıyla papilödemi bulgusu olan ve gecikmiş sol lateral sinüs trombozu olan 22 yaşındabir kadın sunmaktayız. Bu sunumdaki gayemiz genç Afrikalı'larda serebral venöz trombozunKİBAS ve başağrısı ile görülebileceğine ve ayırıcı tanısında bu seçeneğin dışlanmasıgerektiğine dikkat çekmektir. CVST, bu çevrede çok ender olduğundan ayırıcı tanıda sonsıralarda yer almıştır bu hasta için. Bu olgu sunumu Afrika'lı hekimleri bu önemli hastalık içinduyarlı kılmaya yardımcı olacaktır.

Abstract

Headache is a common and protean disorder. The causes of headache range from relativelybenign conditions like migraine, to potentially life threatening causes like rupturedintracranial aneurysms. The diagnostic challenge of headache arises when it occurs inassociation with symptoms characteristic of a wide variety of conditions. Cerebral venoussinus thrombosis (CVST) is a rare form of cerebral vascular accident, occurring in youngadults and women of childbearing age. It presents with headache, signs of raised intracranialpressure and occasionally, focal neurological signs resulting from ischaemia, infarction orheamorrhage into the subarachnoid space. Headache is the commonest presenting symptomand may occur alone, making diagnosis difficult. Diagnosis and management of CVST is alsomade difficult by the numerous risk factors, varied presentation and the absence of anaccepted protocol for treatment. If undiagnosed and untreated, CVST has a high morbidityand mortality rate. We report the case of a 22-year-old woman with Haemoglobin (Hb) ACand Protein S deficiency, who had a delayed diagnosis of left transverse sinus thrombosisfollowing a 2-week history of fever and headache, with papilledema on examination. Ourobjective is to draw attention to the importance of excluding cerebral venous thrombosis inyoung African adults, presenting with headache and features of intracranial hypertension.CVST, being very uncommon in this environment, was low on the list of likely differentialdiagnoses in this patient.This report serves to sensitize the African medical practitioner, to develop a high index ofsuspicion for this important disorder.
