Özet

Lomber omurganın faset eklemlerinin dahil olduğu gut hastalığı ender görülen bir durumdur.Buna bağlı gelişen lomber listezisin neden olduğu kök ve durak kese kompresyonununnörolojik semptomlara neden olması daha da nadir görülür. Hiper-ürisemik gut hikayesi olanve uygun medical tedavi görmemiş 33 yaşında erkek hasta olgusunu sunduk. Bel ve sol bacakağrısı bir aydır artmıştı. Lomber radyolojik incelemeler L5-S1 seviyesinde spondilo-listezisigöstermekteydi. Kontrastli manyetik rezonans görüntüleme (MRG) L4-S1 düzeylerindebilateral, intra-tekal invazyon gösteren ve sol L5 kokunu bası altında tutan paraspinal kitleyigöstermekteydi. Kalp yetmezliğine neden olan Fallot Tetralojisi öykusu nedeniyle cerrahidekompresyon önerilmedi. Bunun yerine local anestezi altında insizyonel biopsy yapıldı.Biopsi materyalinin histolojik incelemesi kronik inflamatuar hücreler tarafından kuşatılmışamorf, eozinofilk, heterojen materyali göstermekteydi. Görünüm gut tofusüyle uyumluydu.Hasta konservatif olarak, non-steoid anti-inflamatuar ve korseyle tedavi edilmiştir. Olgu 16 aytakip edilmiştir. Bel ağrısı devam etmesine karşın günlük aktivitelerine dönebilmiştir.

Abstract

Tophaceous gout involved the facet joints of lumbar spine is rare and the pathologicspodylolisthesis of lumbar spine, presents with neurologic symptoms secondary to spinal cordor nerve root compression remains sketchy. In this study, we reported a case of 33-year-oldmale with the history of hyperuricemic gout without appropriate medical attention. The painin the lower back and left leg radiculopathy was exacerbated over one month. The images oflumbar spine revealed the spodylolytic spondylolisthesis of L5/S1. Enhanced magneticresonance imaging (MRI) showed paraspinal mass at bilateral side of L4/S1 intrathecalinvasion compressing left L5 nerve root. Because of his medial history of Tetralogy of Fallotwith congestive heart failure, surgical decompression for lumbar spinal canal stenosis was notrecommended. Incision biopsy was performed for confirmed the diagnosis by localanesthesia. Histology of biopsy specimen revealed aggregation of pale amorphous andeosinophilic, fibrillary material surrounded by chronic inflammatory cells, foreign giants cellsreaction and fibrosis. The appearance was compatible with gouty tophus. The patient wastreated conservatively with non-steroid anti-inflammatory drugs and lumbar corset. Thepatient was followed for 16 months. Although he still had pain in the low back, but he canreturn to daily activities now.
