Neuroprotective Effects of Clopidogrel in Experimental Spinal Cord Injury

Özet
Giriş: Klopidogrel, platelet agregasyonu vea aktivasyonunu inhibe eden bir ajan olmakla beraber inflamasyon ve vasküler hasara da olumlu etkisi mevcuttur. İnflamasyonu ve oksidatif stresi azaltarak vasküler sistemi koruyucu etkisi bulunmaktadır. Metod: Bu çalışmada spinal kord hasarı olan hayvan modelinde klopidogrel'in etkisini araştırmayı amaçladık. Çalışmamızda toplamada 21 adet sıçan kullanıldı. Kontrol grubunda toplamda 7 sıçana laminektomi uygulandı. Travma grubunda 7 adet sıçana spinal kord hasarı sonrası laminektomi uygulandı.Travma ve klopidogrel grubunda 7 sıçana spinal kord hasarı sonrası laminektomi uygulandı ve gastrik lavaj ile 5mg/kg dozunda klopidogrel verildi. Fonksiyonel iyileşme eğik düzlem testi ve Drummond ve Moore kriterlerine göre değerlendirildi. Elde edilen dokulara histopatolojik değerlendirme de yapıldı. Bulgular: Motor fonksiyon değerlendirmesinde klopidogrel verilmeyen ve verilen travma grupları karşılaştırıldığında yirmibirinci günde fonksiyonel skorlarda istatistiksel olarak anlamlı farklılık saptandı. Klopidogrel verilen grupta verilmeyen gruba göre motor skorlarda iyileşme izlendi. Sonuç: Deneysel spinal kord hasarında klopidogrel'in nörolojik fonksiyonlar üzerine olumlu etkisi olduğu izlenmiştir
Abstract
Background and Aim: The pathological course of spinal cord contusion injuries includes primary and secondary mechanisms of injury. Secondary injury may respond to pharmacologic interventions. The aim of this study was to investigate the effect of clopidogrel on the recovery of neurological functions in a rat model of spinal cord injury (SCI). Material and Methods: The study was performed at Haydarpasa Numune Training Hospital, Istanbul, Turkey. Twenty-one rats were randomly assigned to the following study groups: the control group (n=7), undergoing laminectomy procedure; trauma group (n=7), undergoing spinal cord injury after laminectomy; and trauma plus clopidogrel group, undergoing spinal cord injury after laminectomy and later administration of clopidogrel. Clopidogrel was administered at a dosage of 5mg/kg and functional outcome was evaluated by using inclined plane test and Drummond and Moore motor function score on days 1, 7, 14, 21 after SCI. Spinal cord samples for histopathologic evaluations were obtained after clinical examinations. Results: Inclined plane test and the motor function scores of trauma and trauma plus clopidogrel groups were significantly lower than the control group on days 1, 7, 14, and 21. Inclined plane test of trauma plus clopidogrel group was significantly better than the trauma group on days 14 and 21. In trauma plus clopidogrel group, the motor function scores on day 21 was significantly higher than the trauma group. Conclusion: Our results demonstrated that clopidogrel has a positive impact on neurological functions after injury to the rat spinal cord
