Wernicke Encephalopathy and Central Pontine Myelinolysis: An Underdiagnosed Combination in Alcoholics

Özet
Vernicke Ansefalopatisi (VE) ve santral pontin myelinozis (SPM) birkaç durumda görülen ikiayrı nörolojik tanıdır. Kronik alkolik hastalarda VE ve SPMnin birlikte görülmesi ender birdurumdur. Bu klinik tablo ve radyolojik değişikliklerle ilgili az şey bilinmektedir. BuradaVEnin klasik bulgularıyla birlikte ek olarak hiponatremisi olmadığı halde SPMnin bulgularıolan alkol yoksunluğu halinde kronik alkolik bir olgu sunuyoruz. Tutucu tedaviylesemptomlar bir ay içerisinde tedricen düzelmiştir. Takibin dördüncü ayında elde olunanMRGde pontin lezyonların tamamen kaybolduğu görülmektedir.
Abstract
Wernicke´s encephalopathy (WE) and central pontine myelinolysis (CPM) are two neurologicdiseases associated with several conditions. In patients with chronic alcoholism thecombination of WE and CPM is rare and little is known about its clinical picture and theradiological changes. We report a chronic alcoholic patient, who had classic clinical featuresof WE with additional features of CPM without hyponatremia during alcohol withdrawal. Thesymptoms gradually improved after 1 month with only conservative treatment, and the 4month-follow-up MRI showed complete resolution of the pontine lesions.
